Twisted Squirm is similar to his Toon counterpart, but with a few major changes. Twisted Squirm's general color palette has been desaturated. He resides in a light green broken cocoon, which is covered and filled with Ichor. It hangs from the ceiling using the same substance. His arms have been replaced with Ichor claws, and his antennae have elongated, with Ichor on the tips. Twisted Squirm features the typical red sclerae, black irises, and red pupils that the other Twisteds have. He also appears to be in a perpetual state of sadness, with a watery look to his eyes, and a regretful face should he succeed in attacking a Toon. Twisted Squirm's eyes glow red in Blackouts, making him easier to locate.

Twisted Squirm, unlike all other Twisteds, spawns on the ceiling rather than the ground. Getting too close will trigger him, and he will reach down and grab the player. The player must click side to side (the A and D keys if on PC) to break free from his grasp. If the player fails to do so, Twisted Squirm will take 1 Heart away from the player, and will proceed to relocate himself.

If you are not that good with defending though, looking up most of the time (While avoiding other Twisteds and/or the distractor) can be helpful if you know he is on the Floor. If you see him, simply turn around or find a way to maneuver around him. However, if he hangs above an high rarity Item, a Machine, or blocks off an important pathway, either bite the bullet or have a more skilled teammate ward him off for you. When you see him, you should alert your teammates, as if someone triggers him, it will cause him to relocate (though he does relocate if not provoked for long enough), albeit he makes a sound when doing so.

Some Abilities, such as Connie's Ability Haunting Escape or Goob's Ability Icon Hug!, work when someone or the player themself is grabbed by Twisted Squirm, so they can be useful to save or be saved from one of his attacks. Additionally, Twisted Squirm cannot grab you if you quickly break his line of sight, similar to ranged Twisteds, so if you run into him, just try to turn back. Additionally using a Smoke Bomb immediately discourages him from pouncing you if he already did.

Some Floors, such as the Main Character themed Floors (like the outdoors section of Sprout's Floor or the fossil area of Shelly's Floor), have very tall ceilings. If you know he is there, it could be hard trying to find him, especially if it's a Blackout. However, as long as you can master the clicks, you can easily deal with this.

Do note that during Blackouts, his eyes glow, so finding him will not be that much harder, though you may be more thrown off-guard.

When Twisted Squirm spawns above or close to a bookshelf, he would proceed to eat all the books inside of it, in a similar fashion to his Squirm Render Toon counterpart. Twisted Squirm is the first and currently only Twisted to reside solely on the ceiling, never touching the ground. Due to this, he has unique walking and running animations in Roleplay Mode. However, he isn't the first Twisted to have a unique moving animation in Roleplay Mode, as most of the stationary Twisteds also have this feature. Twisted Squirm has a unique glow effect to his eyes during a Blackout. This is to make him easier to detect, as the light produced by Toons doesn't typically reach the upper portion of maps. This also makes him the first twisted to have their eyes glowing during a Blackout.

Twisted Squirm is the first and currently only Twisted whose render is upside down. This is due to his in-game mechanic. After a certain amount of time of not seeing a Toon, Twisted Squirm will move to another location. There is currently a bug where the moment Twisted Squirm tries to grab someone and they move just out of his reach, he will be stuck. However, moving too far from his range will make him retreat, but not relocate. Even though Twisted Squirm's body is upside down, his tears still flow to his cheeks instead of to his forehead. Twisted Squirm's QTE event becomes more difficult if you keep getting caught by him. Specifically, you start out with needing 20 clicks, with each encounter needing 5 more.
