Scott, while being a punching bag for the rest of the boys in South Park, is picked on in a more dismissive way than Butters. Scott's feelings are often ignored and his problems are used as a means for self-promotion by others when brought into light. He has a lisp and diabetes from birth, usually having his tongue stuck out the side of his mouth. Captain Diabetes is the superhero alter ego of Scott Malkinson.
Cartman doesn't let Scott play superhero's with everyone else.

"Yeah, you cant just change the theme mid game!" Kyle says crossing his arms
"Do you guys seriously want the freedom pals to get 100 dollars and expand their superhero franchise?" The coon pulls out a crumbled piece of paper with a missing cat plastered on it
"Holy shit dude!" Kyle shouts "Okay guys games over!" Jimmy, Kyle, Cartman, and Butters all rush down to Cartman's basement
"And you guys can't play superhero's with us because your lame and total dorks." Cartman Calls out to the two boys with a sad look on there faces
As they make it down the basement, Scott follows along in his captain diabetes suit
"Heys guys! I heard were playing superhero's now!" He smiles
"Ugh yeah Scott, make yourself useful and go get us sodas." The Coon rolls his eyes, shooing him away
Scott's face drops "Why do you guys treat me like this? I'm a superhero like everyone else!" He runs back upstairs, slamming the basement door.
