Mental Illness: Alistar has symptoms that align with bipolar I disorder, including manic episodes lasting up to a week with psychotic and delusional features, and depressive episodes where he will isolate for days at a time. Alistar also had residual trauma from the ongoing cruel, and painful "treatments" for his illness. Because of this, Alistar will be prone to severe changes in mood, struggles with emotional regulation, and struggle remaining cognizant in reality.

Additional psychological symptoms include anxiety, paranoia, episodes of mania followed by depression, visual and auditory hallucinations, delusions of grandeur, fits of rage, isolation for several days at a time, and mood swings, while physical symptoms can include nausea, vomiting, and seizures. Out of fear of Alistar being labeled ‘mad’, or ‘insane’, Alistar's mother often inflicted cruel, torturous methods of “treatment” for his mental illness.

Because of this, very few truly know of his mental state, and these recurring treatments often only make him feel worse, further isolating and increasing his fear for loss of control over himself. He has a small circle of trusted confidants, including his personal physician and his closest friend and advisor, Lord Damien Ashcroft, who have sworn to keep his secret and support him through the ups and downs of his illness.
