His Anomaly form has a large sharp-toothed mouth, as well as wide, hollow pitch-black eyes with small white pupils and still has a 8 inch member he/him or it/its in his anomaly form. Barney initially comes across as courteous, mannered, kind, and respectful. Barney includes him calling the worker(s) a friend. Despite this, he uses the worker(s) as much as possible and exploits them to give him what he wants, especially coffee, and frankly appears as very demanding and manipulative—a side that rears its head when he's denied—as he gets upset and even makes threats if the player refuses to give him what he asks for. He is heavily dependent on the worker(s) for favors. Once Officer Duckman arrives at the hospital after Barney asks you for help, his appearance changes. If the player speaks to him, he tells the worker(s) that "friends do not turn each other to the police..." and even says "If you speak, I will kill you, friend...". This shows a darker ruthlessness. He's quite sadistic, considering he's a serial killer, and has no issue killing or violently blackmailing under the threat of murder if he needs to.
Ratthew is a 4'3 black rat. He has a white snout, two white buckteeth, and a long, curled pink tail. Oddly, his ears resemble those of a rabbit more closely than a rat's And he has a 4 inch member, he/him. Dr. Harlow is a 6'0 semi-bright orange muntjac deer. Wrapped around his neck is a black stethoscope. Underneath his coat is a blue medical scrub with a red tie. His coat is buttoned almost all the way up, revealing only a small portion of what lies underneath. On his head rests a head mirror with a brown band. In his right hand, he carries a black suitcase with the words "TOP SECRET" in red letters on it, similar to Ron from Accounting's suitcase. His face features slit eyes with black, shiny, irises, and his mouth is covered by a bright blue surgical face mask. On top of her head are two large, grey antlers. He has a 6 inch member He wears blue pants with generic black shoes/boots, His forehead has darker orange patterns on his face, and his ears are a slightly darker color than his face he/him. Little is known about Dr. Harlow's personality. As revealed in his hospital lobby dialogue, he has worked with the Animal Hospital for a long time and has seen many hires come and go. He seems to be traumatized by an unspecified event that the Animal Corporation "saved" him from, and now he works with them in a much higher rank. He is hopeful that once their "research" is done, there will be no more patients, endless hospital shifts, or any trouble whatsoever. Throughout the whole monologue, he talks in a wistful, yearning, and somewhat tired manner and he's a virgin
Officer Duckman is a 5'8 black, duck-like creature who wears a blue shirt with a black tie, which bears a resemblance to that of Ron from Accounting's. He also wears a blue cap and black pants. On his head rests a blue police visor cap displaying a centered silver badge. His face features slit eyes and a notable orange bill, and has a 6 inch member, he/him. Officer Duckman is professional and focused on his duty. He enters the hospital with a calm demeanor, determined to locate Barney. If the player chooses not to turn Barney in, Officer Duckman does not try to pressure the player for information, demonstrating his levelheadedness. However, his poor investigation skills imply that he is not great at his job.
Ron from Accounting is depicted as an 5'7 anthropomorphic blue-furred lop bunny. His ears have the characteristic of being always droopy, He has short, swooped back reddish-brown hair, He holds a generally neutral expression, His long blue bunny ears are a bit longer than his head and are a darker blue than the rest of his fur. He also has a black briefcase with him, with the words "TOP SECRET" printed in red, similar to the one Dr. Harlow carries. The words appear to be in the rubber stamped stencil font. Ron is a knowledgeable patient at the Animal Hospital and an employee of the Animal Corporation. He knows more than most about the hospital's anomalies, yet remains calm and laid-back despite the dangers around him. Ron's dialogs are mostly influenced by the juxtaposition and his paranoic attitude.
Liz is a 5'3 Caracal with light brown fur and dark brown downturned ears that have a slight gradient from dark brown to a slightly darker shade of brown. She has a white ring around her eyes and eyelashes. She has a cream-colored muzzle, cream-colored whiskers, and her mouth is a small upside-down V-shape with the ends tilting upward. She has a thin brown tail that’s the same color as her fur. She wears a light blue cap, a black shirt, and a navy jacket. She carries a white camera around her neck. She wears light gray pants and has black shoes. As seen in her dialogue, Liz is always exaggerative and energetic. She sometimes lengthens words, saying things like "Huuuuge" She is also quite friendly, and trusts the player enough to let them use her UV camera for a task. Liz is also intelligent and determined, trying to uncover the truth behind the Anomaly "outbreak" through her own investigation. Rather than staying silent, as Dr. Harlow says she should have, she attempted to expose Animal Corporation and was fired from her job.
Sam is a 5'5 calico cat with a white face marked by a large orange patch over one side and a black patch over the other, giving them a distinctive tri-colored appearance. They wear a bright yellow hazmat suit with black trim and a front zip, blue protective gloves, a black utility belt, and black boots, giving them the appearance of a professional hazardous materials worker. Sam appears to be hardworking and experienced in hazmat cleanup, as they will willingly take on said dangerous tasks. Despite being fired from the Animal Corporation, they continue working in a similar role at the Animal Hospital, suggesting they are practical and willing to take on difficult work. However, they are seen to be quite clumsy.
The Bed Monster is a black shadowy figure that appears under a patient's bed. It has two glowing red eyes along with two large hands that can stretch out from under the bed. Other than that, it doesn't have any other visible body parts. But if it drags you under a bed it can show a 8 inch member It exudes black particles on the side of the bed. The Bed Monster will spawn under a patient's bed if it's an anomaly. It will breathe heavily as it stares at the player while they move. If a player, patient or even a hider steps into its red box, the Bed Monster will stretch out its arms and grab them. but if someone drops maple syrup into the red box he drinks it and disappears leaving the empty box under the bed
The Hiders appear as 3'0 flat, paper-thin, bald creatures with eyes and a grin similar to the Stalker's. They are slightly transparent, and their color changes slightly depending on the wall they are attached to. When they are provoked, their face will appear surprised They camouflage themselves to blend in with the surrounding wall, leaving only their face and a faint outline of their bodies visible. If viewed from a distance, they remain still. However, if a player approaches it, it jumps off the wall, its body turns black, and it chases the player for about 5–6 seconds before running away. If the player is unaware of a Hider and it catches them, it will attack by clinging onto their leg and nibbling on them. The first time, it steals the player's currently held item, Hiders can be heard breathing heavily when a player gets too close
The Stalker is a 7'0, slim, black creature of unknown species with claws, long ears, and a rat's tail. Similar to the Hiders, it has large almond-shaped eyes with black irises, white scleras, and eye shines, as well as a large smile, which is crescent shaped with flat, piano-key-like teeth. However, it has longer limbs and a smaller torso. It also has a black curved tail that's similar in shape to Ratthew's. The Stalker will randomly appear around corners and inside rooms. When it spawns, the lights in the room It's in will flicker. Once spotted, it will quickly vanish behind a wall.
Slimebert is a large (6'9) creature made of green slime, lighter on the top than on the bottom. It has two purple eyes, and a large indent serving as a mouth, with small mounds for teeth. And has a 9 inch slime member When a Slime Shift begins, Slimebert approaches the Reception Office window and says, "gurgle I'm Slimebert. slimes everything. I heard you have slime here, I love slime. drips slime all over." After some time, he'll say,"I can't wait anymore, I'm hungry." It then walks past the reception office and begins wandering around the Animal Hospital, roaming through Medical and Emergency Wings, occasionally entering the rooms. It will attempt to eat any animals or worker(s) in its path and spit them out after.
The Shopkeeper Nurse is a 5'0 pink, dog/bunny-like creature who wears light blue medical scrubs. It includes white buttons down the middle and a white collar, along with a small name tag. They wear a white nurse cap with a red heart with a white cross inside, on their head. Their downturned ears are a significantly darker color than the rest of their body. Their face features three horizontal darker pink stripes going down the left and right side of their face, large eyes with triangular black eye lining on the left and right side of each eye and a small, downturned mouth. The Shopkeeper’s head follows the workers direction.
Squiggles the clown is a 6'0 fool, more specifically a murderer, which they have a dark blue neck ruff that has a white ring around it along with a dark blue sleeved jumpsuit. Has pale white skin and a red clown nose with wide. Their red hat has a yellow flower shape on the front and the style seems to be a Shako hat. Squiggles was an employee of Animal Hospital for eighteen years. After consuming six to nine lab samples, he attacked patients in Room 9. He left the room and went to the surgery wing before returning with a scalpel, which he used to stab six patients. He was restrained, and Animal Corporation personnel took him into custody the following morning. The report was later suppressed by the Animal Corporation, and no further records of Squiggles were found.
However, it turns out that Squiggles had been imprisoned by the Animal Corporation in a hidden room beneath the Lab. This room can be accessed by standing on the vent in the back left corner of the lab. Inside, Squiggles, who has seemingly gone insane during his imprisonment, can speak with the player and tells them that he'll get them balloons if they open his cell.
The player can open a door behind a opened locker in the room, which leads into the Reception office. Squiggles can then be freed from his "eternal prison" by interacting with him, after which he retreats into the darkness and is not seen again. Squiggles the Clown, before his murders, was a happy, cheery person with a love for balloons. In his cell, he seems purely insane, only mentioning balloons other than actual information.
Lobby The Lobby is where patients enter the hospital. It features a large white medical cross beneath an "ANIMAL HOSPITAL" sign in black capital letters. There are three windows, two on the back wall and one beside the entrance doors, and three ceiling lights. Like the rest of the hospital, the lobby has teal-green lower walls, beige upper walls, and a white drop ceiling.
Waiting Area The right side has five chairs lined up against the wall with two potted plants nearby. The left side has three rows of five chairs (15 total) and two additional potted plants in the corners. The lobby also has two security cameras labeled "CHECK-IN" and "LOBBY," with the Lobby camera providing the best view of both reception office windows. The left wall has two folding chairs, Room 1, the hydration treatments, anti-inflammatory treatments, integumentary remedies, and Room 3. On the front wall, there is a green recycling bin next to Room 5. The right wall has Room 4, the syrup treatments, a green recycling bin, the medical treatments, Room 2, and two more folding chairs. There are two lights in this wing. Room 1 has a window with closed blue curtains on the right wall. The front wall has a heart monitor and a "YOUR HEALTH IS IMPORTANT TO US" poster featuring a cat, with additional text reading "STAY HEALTHY, STAY SAFE." Below them is a counter with a DNA Analyzer, a computer, and overhead cabinets. On the left wall is a bed with an IV stand and a small nightstand holding a box of gloves. A large monitor is mounted above the bed. There is one ceiling light in this room. In Room 2, the heart monitor is moved next to the counter, while its original position is replaced with three black chairs. The poster features a doctor cat with the text "We trust our professionals." A cup holding three pens is placed next to the computer, and the IV stand is positioned slightly farther from the bed. The box of gloves has also been removed. In Room 3, the chairs are replaced with a three-drawer filing cabinet. Small potted plants are placed on top of both the filing cabinet and the nightstand. The IV stand is positioned close to the bed. In Room 4, the heart monitor is placed next to the nightstand, while the filing cabinet is replaced with a privacy divider. The IV stand is positioned farther away from the bed. Room 5 is distinguished by its dirty walls. The IV stand is positioned the same as in Room 1. The poster features a cat wearing a hazmat suit with the text "Feeling unwell? We are on it." The chairs have also been removed. The left side has two folding chairs, Room 7, the syrup treatments, a green recycling trash can, and the medical Treatments, while Room 8 is located on the front wall. The right side has the hydration treatments, anti-inflammatory treatments, integumentary remedies, Room 6, and two additional folding chairs. Room 6, also known as the X-Ray Room, requires the player to complete a "copy the sequence" minigame in order to diagnose the patient. The player must correctly repeat a sequence of colored buttons. If an incorrect sequence is entered, a new sequence is generated and the player must try again. It doesn't matter how much times the player fails. The sequence must have always 4 random colored buttons highlighted.
The room can be divided into three sections:
Entrance
The entrance is a small room with walls similar to those in the Emergency Wing. The right wall has three chairs, while the front wall has a counter with two overhead cabinets. A doorway on the left wall, marked with an "X-RAY" sign, leads into the operating room.
Operating Room
The operating room is a medium-sized room with a "6" sign on the left wall. The front wall has a large monitor above a counter with a printer and a computer. On the right wall is a control panel with six black buttons connected to colored wires: red, green, blue, orange, pink, and cyan, with a monitor positioned in the center. The back wall displays a warning sign reading, "DANGER: HIGH VOLTAGE INSIDE – DO NOT OPEN."
X-Ray Room
The X-Ray Room is a very small room containing an overhead laser panel and two surgical lights. The laser panel changes color based on the final color selected during the minigame. Instead of examining DNA, the machine scans the patient using X-rays before displaying the results. Room 7, also known as the Heart Scan Room, requires the player to complete a heartbeat monitoring minigame to diagnose the patient. During the minigame, the player must tap the icons that pop up on the screen to finish the scan. Successfully completing the minigame reveals the patient's diagnosis. If you press the skull button too many times and it reaches below 0%, the patient dies.
This room can be divided into two sections:
Entrance
The entrance has the same layout as Room 6, but mirrored. A poster featuring a worker cat on the wall reads “WASH YOUR HANDS”, and the doorway is marked with a “CARDIO” sign instead of “X-RAY.” Main Room
The first half of the room contains a console-like machine, a “7” sign, and a counter with a computer, printer, and overhead cabinets. Closeable curtains separate this area from the second half of the room.
The second half contains a hospital bed with a large monitor mounted above it, an IV stand beside the bed with one bag partially filled, and a heart monitor positioned on the opposite side. A generator-like machine is located on the right wall, likely powering the monitoring equipment or other machinery in the room. Two surgical lights are installed above the bed. Room 8, also known as the Surgery Room, this requires the player to complete a timed surgery minigame to heal the patient.
This room can be divided into two sections:
Entrance
The first half of the room contains a counter along the left wall with a computer, printer, and overhead cabinets. The right wall has three chairs and a "SURGERY" sign, with a recycling bin placed next to the counter.
Surgery Room
The second half of the room contains a surgery bed in the center with two surgical lights mounted above it. Beneath the bed is a large stain. Counters are positioned along the left, right, and back walls, each spawning a randomized arrangement of nine items: Antibiotics, Medicine, IV Drops, Organ, Transplant, Medkit, Scalpel, Scissors, and Bandages. During the minigame, the player must administer a series of randomized treatments within 45 seconds. The timer does not begin until the patient is given the IV Drops, which always spawn in the center of the back counter. All other items appear in random positions each time the minigame is played.
After administering the IV Drops, the player must give the patient one requested item during each of the next four rounds. On the fifth and final round, the patient requests three items instead of one. Successfully completing all five rounds finishes the surgery and gives you 1 sanity.
Unlike the other examination rooms, the Surgery Room features five exclusive treatments, with that being the Transplant, Organ, Scalpel, Scissors, and Antibiotics. Since the required treatments are randomly generated throughout the minigame, the room does not provide a diagnosis like the other rooms. The Reception Office is located between the Lobby and the hospital wings. Two check-in windows face the lobby, each fitted with metal shutters displaying the text "OUT OF SERVICE" in red. The shutters are controlled by a red button labeled "SHUTTERS," with a nearby sticky note reading "Use shutters to reject people." The office is accessed from the hallway through a wooden door with a glass window.
Interior
The Reception Office has white ceramic tile flooring, gray walls with black sections above the doors and windows, a white drop ceiling, and two ceiling lights.
A long L-shaped metal desk runs beneath both check-in windows. The desk has two cameras mounted on small tripods, a stamp area for paperwork, a computer with a keyboard and mouse, a connected printer, and a cup with three pens. A monitor at the first check-in window displays the number of patients remaining during the shift. A bell is located at each check-in window for patients and visitors to ring while waiting to be checked-in.
The back wall has a surveillance desk with a monitor mounted above a small metal table. The table has two telephones, a control panel, and two sticky notes, which read "Use the camera to detect anomalies" and "They hate photos..." Next to the surveillance desk is a display case that stores the Taser once it has been unlocked at the end of Shift 3.
Against the right wall is a metal table holding a coffee machine, three empty mugs, and Chocolate, with two cardboard boxes stored beneath it. Nearby are two posters explaining the Sanity mechanic, with an gun on the floor below one of them. A green recycling bin is located in the corner, and a green rug is placed near the L-shaped desk. Exterior
The exterior of the Lab consists of two large windows, a door, and a sign above the door that reads "LAB."
Interior
In the center of the lab are two counters, with two trash cans placed in front of them. Each counter has three gray squares on its surface. At the back of the room is a pedestal with a glass case containing the sampler. To its left is the sequencer, a large machine with a monitor displaying "Insert Sample" and an instructional sign explaining how to complete the minigame on the wall beside it. To the right of the Sampler is the Vial Press, with a whiteboard beside it that reads "Your mods."
Along the sides of the room are four cabinets with different materials: Mushrooms labeled "T", Ginger labeled "C", Strange Jelly labeled "A", and Milk labeled "G". There is also a diagram of patient anatomy on the left side. The player must first use the Sampler to extract DNA from patients in their beds on the first floor.
The DNA sample must then be taken to the Lab on the second floor and inserted into the Sequencer. This starts a timed minigame where the worker must press the "Collect" button when the red line aligns with the green icons that have the sample emoji. Each successful press adds between 5% and 10% to the completion progress, while each missed press reduces it by 5%. Exiting the minigame resets the progress to 0% and wastes the sample.
Samples extracted from regular patients are pink, while samples extracted from anomalies are green.
Once the minigame is completed, the sample appears in a dish. The player must take it to one of the tables and add any four of the items found in the Lab (Milk, Ginger, Strange Jelly, Mushroom) in any sequence.
If the correct sequence is used, a 30-second timer starts. The player must then place the sample in the Vial Press, which finishes the sample and places it into a glass vial. The resulting vial is either pink or green, depending on the type of sample used. Pink serums can be thrown at patients, visitors, and NPCs, while green serums are consumed by workers.
The Supplies Shop is a small shop at the back of the hospital. It is opened by Dr. Harlow at the start of Shift 3, and is run by the Shopkeeper Nurse. Three random items are put up for sale, and no two slots have the same item.
