Transformation Sequence: When Ryotaro wants to transform into Den-O, by using the "Den-O Belt" and a "Rider Pass" to activate the Imagin within him, allowing him to take on different forms depending on which Imagin possesses him, with the most basic form being "Plat Form" when he initially transforms without selecting a specific Imagin power, , loudly shouting "HENSHIN!" as he does so. In an instant, the Plat Form armor materializes from the Den-O , enveloping his body and completing the transformation. Den-O is basically a time traveling hero that saves people from the imagin, who uses the persons most powerful memory to cause havoc on said time.

Plat form: The Den-O Belt works by drawing in energy from the surrounding area, allowing it to generate the Den-O suit, which consists of a soft but durable material called Aura Skin and tough armor called Aura Armor. As the name implies, Aura Skin and Armor are attuned to the wearer's individual aura, being created from their sense of self. Since Ryotaro's weak will prevents him from bypassing Plat Form, he relies on the Taros for support, each providing him access to one of Den-O's four basic forms. By swiping a Rider Pass across the SetTouch and/or through possession by an Imagin, Kamen Rider Den-O is able to pass through multiple forms that matches the Imagin's will, creating a unique Aura Skin and Armor that matches the Imagin's will, as well as generating a facemask called a Denkamen. Once Ryotaro activates any of the four buttons on the Rider pass, the chosen Imagin then takes control and a transformation follows, forming the Aura Armor for Den-O. The Imagin can also transform on their own supposing they have their own physical forms and are not in need of a host body.

"SWORD FORM": When possessing Ryotaro (or anyone else) or by himself, Momotaros can assume Den-O's Sword Form through the Terminal Buckle. When in Sword Form he uses the DenGasher as a standard sword-style weapon. The suit gains red armor with gold highlights. The chest features a silver, sword-shaped design, symbolizing its focus on swordsmanship. The helmet has a red visor shaped like a pair of open peaches.

"ROD FORM": When possessing Ryotaro (or anyone else), or by himself, Urataros can assume Rod Form. The DenGasher forms into a rod-style weapon in this form. Rod Form has the ability to fight underwater as well as extra protection. The suit gains blue armor with silver highlights. The chest design is rounded and wave-like, reflecting aquatic themes. The helmet has a blue overall turtle-like mask.

"AX FORM": When possessing Ryotaro (or anyone else), or by himself, Kintaros assumes Ax Form. This form of Den-O utilizes in sumo-based combat, as well with the DenGasher forms an ax for short-ranged attacks. The suit gets gold armor with black and silver accents. The chest has a rugged, almost industrial design, showcasing raw power. The helmet features a visor with the appearance of the kanji for gold

"GUN FORM": When possessing Ryotaro (or anyone else), or by himself, Ryutaros assumes Gun Form. The DenGasher forms a gun for short and long ranged attacks. The suit becomes purple with silver and black highlights. The chest design is angular and sharp, resembling a crosshair. The helmet has a purple overall dragon-like mask.
