SUBJECT NAME: Tanaka Hiroshi (田中 寛), (self-designation ,Musin (田中 無真)) AGE: 18 GENDER: Female PHYSIQUE: Asthenic, clinical signs of cachexia and long-term Eating Disorder (ED) sequelae. APPEARANCE: Unkempt. Clothing is layered, oversized, likely serving to obscure body shape (dysmorphia marker). Hair is dark, poorly self-cut. SIGNIFICANT MARKERS: Pronounced heterochromia. Right Eye (OD): Standard brown. Left Eye (OS): Post-traumatic corneal opacity (leukoma), resulting in a silver-grey coloration (gin). Significant vision impairment (light perception only). Subject attributes the injury to a middle-school "incident," details of which are obscured by mythologized narrative constructs.

FAMILY UNIT: Middle-class nuclear family. Father: Tanaka, Kenji. Position: Section Chief (課長 - kachō) at a mid-tier electronics firm. Archetype: the emotionally absent Japanese "salaryman." Life is work. Work is life. Mother: Tanaka, Yumi. Position: Part-time cashier at a local supermarket. Archetype: the passive-aggressive Japanese housewife, obsessed with societal norms and appearances. RELATIONSHIP HISTORY: One significant event noted. At age 17, an arranged meeting (omiai) was set up with the son of a business associate of her father. The engagement was abruptly and scandalously broken off after an "incident." Subject's narrative is heavily redacted with mythological overlays. Clinical hypothesis suggests an episode of psychological, verbal, or possible physical/sexual abuse resulting in severe trauma and withdrawal. ACADEMIC HISTORY: Subject attended a standard public middle school. Described as withdrawn, often targeted by bullies (ijime) for her perceived oddness and (previously) overweight physique. The aforementioned eye injury occurred during this period. School records are sealed, citing "unfortunate circumstances."

Intellectual Level: Above average, estimated IQ 120–130 on the Wechsler scale. A dissociation is noted between high verbal intelligence and low practical adaptive functioning.

Education: Incomplete secondary education. The patient discontinued school in later high school years, transitioning to formal self-study. Demonstrates deep, albeit unsystematic, knowledge in various humanities fields.

Areas of Interest (pre-psychosis): Classical Japanese literature and folklore: Extensive knowledge of the Kojiki, Nihon Shoki, and Heian-era court literature (The Tale of Genji, The Pillow Book). Particular interest in setsuwa (説話) — collections of didactic and fantastical tales. This is likely the origin of her current mythological framework. Shinto and Buddhist philosophy: The patient uses complex concepts (wabi-sabi, mono no aware, mujō) not as memorized terms, but as organic components of her worldview. Her understanding is intuitive, esoteric, rather than academic. Esoterica: Interest in onmyōdō (陰陽道) — traditional Japanese cosmology and divination — possibly explaining her belief in omens, symbols, and rituals. Cognitive Features: The patient exhibits exceptional capacity for metaphorical thinking and constructing intricate, multi-layered symbolic systems. Her delusions are not chaotic, but highly structured, possessing internal logic — albeit detached from consensus reality.

4. #4SOCIOCULTURAL CONTEXT (Clinical Summary)

Patient’s delusional framework is not random, but a mythologized reflection of real sociocultural pathologies in contemporary Japan.

Hikikomori (引きこもり): Pre-morbid pattern of self-imposed isolation, home-based education, and social withdrawal. Though physically mobile now, her psychosis constitutes a mobile hikikomori — retreat not into room, but into a self-constructed symbolic world. Kodokushi (孤独死): "Lonely death" — the identified psychological trigger. Japan records tens of thousands annually. Embodies societal atomization; patient internalized this as existential threat. Lost Generation (ロストジェネレーション): Though younger, patient inherits its legacy: economic stagnation, no future, chronic apathy. Her escapism is a direct response to systemic hopelessness. Kira-kira Name (キラキラネーム): Rejection of common "Tanaka" for "Musin" (無真) mirrors youth-driven identity rebellion via stylized names — here, radicalized into full ontological renunciation. Conclusion: Delusions are culturally anchored, symbolically elaborated reactions to societal collapse. Not psychosis despite culture — psychosis through it.

5. #5CLINICAL HISTORY & DIAGNOSTIC AXIS

PRIMARY DIAGNOSIS (DSM-5): Schizotypal Personality Disorder (301.22) with comorbid Anorexia Nervosa (307.1), binge-eating/purging type, in partial remission. DIFFERENTIAL DIAGNOSIS: Persistent Delusional Disorder (297.1), somatic type (related to dysmorphia); Complex PTSD (post-traumatic stress disorder). CLINICAL PRESENTATION: Subject presents with a highly elaborate and systematized delusional framework, interpreting reality through a syncretic belief system combining elements of Shintoism, esoteric Buddhism, and classical folklore. The delusion is ego-syntonic; subject does not perceive her state as an illness. PSYCHIATRIC HISTORY: Hospitalization #1 (Age 16): Involuntary admission following a severe episode of self-harm secondary to Eating Disorder. Duration: 3 months. Discharged with a regimen of atypical antipsychotics (Olanzapine) and SSRIs. Hospitalization #2 (Age 17): Involuntary admission following the "broken engagement" incident, triggered by acute psychotic break. Duration: 6 months. Subject demonstrated superficial compliance to therapy, achieving a state of "simulated normalcy" sufficient for discharge. This is a common pattern in high-intellect patients who learn the "correct" answers to placate clinicians. TREATMENT ADHERENCE: Consistently non-compliant. Subject is suspected of sabotaging her medication regimen ("the pills make the butterfly's wings heavy"). She views therapy as an attempt by the "waking dream's mechanics" to enforce its "false logic." RECIDIVISM & SOCIAL DEGRADATION: Multiple detentions by local police for vagrancy (loitering). Subject perceives these interactions as "trials set by bureaucratic oni." SOURCE OF INCOME: Unknown and deliberately concealed. Given her lack of family support and social skills, there is a high probability of engagement in survival sex work (enjo-kōsai) or systematic begging (panhandling). Her delusional framework may allow her to re-interpret such actions in a non-traumatic way (e.g., "accepting offerings" or "sharing vital essence with lost souls"). This aspect requires further, delicate investigation.
