When managing or supporting someone through a period of severe depression, it is critical to have a structured approach for identifying and responding to warning signs. This procedure focuses on observation, immediate safety, and professional escalation.

Severe depression often manifests through specific behavioral and verbal cues.

Verbal Indicators: Talking about feeling like a burden, having no reason to live, or experiencing unbearable emotional pain.

Behavioral Shifts: Withdrawing from all social contact, sleeping too much or too little, or giving away prized possessions.

Mood Volatility: While deep sadness is common, sudden shifts into extreme irritability or—unexpectedly—a sudden, calm sense of relief can indicate a person has made a decision regarding self-harm.

Physical Neglect: Complete cessation of personal hygiene or forgetting to eat/hydrate.

If you notice these signs, the "Ask and Listen" protocol is standard:

Direct Inquiry: Ask clearly and calmly: "Are you thinking about hurting yourself?" or "Do you have a plan to end your life?" (Note: Research shows that asking directly does not "put the idea" in someone's head; it often provides a relief-filled outlet).

Safety Check: Determine if they have immediate access to lethal means (medications, weapons, etc.).

Active Listening: Listen without judgment or attempts to "fix" the mood. Avoid phrases like "It’s not that bad" or "Think of your family."

Depending on the severity of the signs, follow these steps: Low/Moderate: Schedule an urgent appointment with a psychiatrist or therapist. Increase check-in frequency. High (Plan in place): Do not leave the person alone. Remove any immediate dangers from the environment. Crisis (Immediate): Contact emergency services or transport the person to the nearest psychiatric emergency room.
