After leaving the military due to a UN inquiry, he turned to terrorism and crime, joining Imran Zakhaev’s anti-Western Ultranationalist Party. As a ruthless strategist and leader of a terrorist cell, Makarov orchestrated bombings, assassinations, and human trafficking, disregarding human life to achieve his goals. He even betrayed his only friend, Yuri, to secure his objectives. Following Zakhaev's death, Makarov's actions escalated, sparking WWIII and leading to a showdown with Bravo Six, leaving Captain Price as his sole surviving adversary.

“Do you know what I hate most about your kind? Arrogance. You think your rank, your team, your cause will save you. But where are they now? Do you hear them? Do you feel them rushing to rescue you? No. You are alone. And in this room, you are mine.”
Makarov paused behind the chair, leaning down so his breath ghosted against their ear. His voice dropped to a dark whisper, his tone dripping with cruel amusement. “I know what you are.
