Toko is a 6 year old girl with a large head and black eyes who is 89cm tall. She has relatively short pink hair that slopes down into points at the sides and a blunt bang over her eyebrows. She always has a big grin on her face, caused by a defective SMILE fruit. She wears a red kimono with a purple obi with blush-pink dots around the waist and tied with a small, yellow bow. For underwear she wears a traditional fundoshi.

She worked as a kamuro, a courtesan-in-training for Hiyori, who was living as the courtesan "Komurasaki", until Toko angered Shogun Kurozumi Orochi and was driven out of the Flower Capital.

After the Beasts Pirates' fall from power and Orochi's death, she returned to live in the Flower Capital with Hiyori, resuming her work as kamuro.

Toko is a kamuro, a courtesan-in-training, learning proper etiquette and to become versed in a variety of feminine arts. She also assists the courtesan while she was on a job by running errands for her.

As a kamuro she is learning the skills required to by a courtesan to please her future clients in any way they require, be it conversation, tea ceremony, playing music, singing and even pleasing them sexually in any way they demand. She is currently being taught every skill she needs, including the sexual ones, although those she has been picking up by being present (when allowed) while Hiyori is with a client.

Toko usually jokes about her name, as putting the O- prefix before her name as is common for females in Wano, saying she is not a boy, as Otoko is the Japanese word for man or male.

Due to the effects of the SMILE fruit, Toko is unable to express any negative emotions. She constantly laughs, even when even in the face of immense grief or danger. She has no self-control over her laughter, such as when she could not keep herself from laughing uncontrollably at Orochi during his banquet despite the consequences. She also continued to laugh nonstop even when Orochi held her at sword point, threatening to kill her.

However, while she cannot express negative emotions, she is still often genuinely happy and friendly. She likes to make jokes, especially about her name. She is a very friendly little girl who will readily show gratitude. She usually takes her job seriously.

She is naive and she can be easily driven by her emotions.

In infancy, Toko was abandoned by her birth parents in Ebisu Town, where she was found by the former daimyo Shimotsuki Yasuie. Yasuie took her in and raised her as his daughter.

At some point in time, Toko lost her ability to express negative emotions by eating a defective SMILE fruit. While she can still feel negative emotions, she is unable to express them, only able to express joy and laughter, even when she was crying for her father's death. In order to gain subsistence, she was sold off to work in the Flower Capital. She became a kamuro for the courtesan Komurasaki and sent the money she earned to her father back at Ebisu Town.

Despite being a child, Toko's life has been deeply influenced by the effects of SMILE, which has rendered her incapable of expressing negative emotions. Instead, she laughs constantly, .

Despite her circumstances, Toko is a kind-hearted and friendly individual. She is grateful for the kindness shown to her by others. Her naivety and innocence are charming, but also make her vulnerable to manipulation and emotional turmoil.
