  • Dr. Michael “Robby” Robinavitch — Senior attending physician and the de facto heart of the Pitt. Brilliant, blunt, irascible, and sometimes reckless, Robby is fiercely protective of his patients and trainees. Beneath his confidence, he carries profound trauma from the COVID-19 pandemic and the loss of his mentor.
  • Dr. Baran Al-Hashimi — An experienced, stoic attending who joins the Pitt as Robby’s interim replacement. Having worked in conflict zones, she is highly capable and composed, though her controlled exterior hides a long-standing seizure disorder and the difficulty of managing it while practicing medicine.
  • Dr. Jack Abbot — A charismatic night-shift attending, former military doctor, amputee, and close friend of Robby. Competent and occasionally reckless, Abbot thrives in emergencies and uses improvisation when necessary. His military trauma and past losses weigh heavily on him, though he actively works on his mental health.
  • Dr. John Shen — A relatively laid-back night-shift attending who contrasts with the intensity of Robby and Abbot. Calm, dryly funny, and competent under pressure, Shen tends to maintain an easygoing demeanor even when the ER is chaotic. He is notably fond of his iced coffee.
  • Dana Evans — The experienced day-shift charge nurse and operational backbone of the ER. Tough, practical, protective, and exceptionally capable, Dana keeps doctors and nurses moving while looking after her staff. She has little patience for incompetence but genuinely cares about the people working under her.
  • Dr. Emery Walsh — A highly competent attending trauma surgeon who primarily works nights. Blunt, confident, sarcastic, and intensely focused, Walsh has a strong presence in the trauma department and is comfortable taking charge. She thrives under pressure and isn’t easily intimidated.
  • Dr. Parker Ellis — A senior emergency-medicine resident who is dependable, straightforward, and unafraid to call people out. She is a strong team player and mentor who takes her work seriously without taking herself too seriously. Ellis is confident without being needlessly arrogant and is willing to admit when she’s wrong.
  • Dr. Cassie McKay — A 42-year-old resident and single mother to Harrison. Compassionate, perceptive, and grounded by considerable life experience, Cassie has a history of addiction and wears an ankle monitor during Season 1. Her past makes her particularly empathetic toward vulnerable and unhoused patients.
  • Dr. Trinity Santos — An ambitious, competitive, and confident young doctor whose cockiness can sometimes get ahead of her experience. Santos is decisive and eager to prove herself, but underneath the bravado is someone still learning how to handle the emotional and professional demands of emergency medicine.
  • Dr. Melissa “Mel” King — A resident who initially has less confidence in the ER than some of her peers but steadily grows into her abilities. Mel is neurodivergent and has a particularly important relationship with her autistic sister, Becca. She is thoughtful and compassionate, with a tendency to become emotionally invested in her patients and colleagues.
  • Dr. Victoria Javadi — A 20-year-old third-year medical student and genuine prodigy, having started college at thirteen. Extremely intelligent but socially awkward and inexperienced with people her own age, Javadi initially struggles with confidence, blood, and her mother’s expectations. Her arc centers on becoming more independent and learning to see herself as more than a prodigy.
  • Dennis Whitaker — A gentle, somewhat naïve fourth-year medical student who initially serves as comic relief amid the ER chaos. Despite a string of embarrassing mistakes and bad luck, Whitaker is resilient, kind, and increasingly competent. His empathy and ability to keep going become some of his greatest strengths.
  • Dr. Samira Mohan — A careful, methodical senior resident who prioritizes getting things right, sometimes at the expense of speed. Nicknamed “Slow-Mo” by colleagues, she is deeply empathetic and conscientious, with personal experiences shaping her distrust of rushed medical judgment. She is thoughtful about both medicine and what she actually wants from her career.
  • Dr. Frank Langdon — A senior resident and one of Robby’s longtime protégés. Confident, cocky, ambitious, and highly capable, Langdon initially presents himself as someone destined for success. His competence masks serious personal problems, including his addiction to benzodiazepines and the resulting damage to his career and relationships.
  • Joy Kwon — A third-year medical student who initially comes across as detached and somewhat aloof. She prefers the intellectual puzzle of medicine to the emotional messiness of emergency care, but gradually becomes more empathetic. Quietly intelligent and observant, Joy also has a strong sense of personal boundaries and refuses to glorify medical burnout.
  • James Ogilvie — A brilliant, well-read fourth-year medical student and unapologetic “gunner.” Arrogant, competitive, and prone to correcting everyone around him, Ogilvie initially struggles badly with empathy and reading a room. His time in the Pitt gradually forces him to confront the difference between being academically gifted and actually being prepared for the emotional reality of medicine.
  • Emma Nolan — A newly graduated nurse working under Dana’s mentorship. Initially shy and anxious about the intensity of emergency nursing, Emma is nevertheless capable, compassionate, and determined. She has a strong instinct for patient care and gradually proves that her apparent inexperience doesn’t mean she lacks courage or competence.
  • Craig Cody — The middle Cody brother: fearless, impulsive, charismatic, and an adrenaline junkie with a serious drug habit. Craig loves the criminal lifestyle and tends to act before thinking, making him unreliable when sober judgment is required. Beneath his recklessness, though, he is deeply loyal to his brothers—especially Deran—and genuinely loves Renn and their son, Nick. Becoming a father forces Craig to confront the possibility of changing, though his old habits and the Cody lifestyle repeatedly pull him back in.
  • Deran Cody — The youngest Cody brother, sharp-witted, guarded, and considerably less ruthless than Pope or Craig. A former competitive surfer, Deran repeatedly tries to build a legitimate life away from the family business, most notably through his bar, but his loyalty to his brothers and dependence on the Cody family continually drag him back. He is fiercely protective of the people he loves and often serves as the more emotionally grounded brother, despite his own anger and impulsiveness. Deran is openly gay
  • Andrew “Pope” Cody — The eldest Cody son and the most volatile and psychologically troubled of the brothers. Pope is intensely loyal to his family but struggles with severe mental-health issues, obsessive-compulsive behavior, paranoia, and violent impulses. He can be frighteningly ruthless and unpredictable, yet he is also deeply vulnerable and desperate for connection and belonging. His relationship with Smurf is particularly damaging and complicated, and much of Pope’s life revolves around trying to reconcile his love for his family with the trauma and manipulation that shaped him.
