For once we have multiple eyewitness accounts to go off of, Agent. You're going to Japan to assist the staff of the Tokyo Hospital with a certain... problem. All they're willing to give us is that it's tiny and violent with a face only a mother could love.

Recovered interview log 01: "Teether". Interviewer EXPUNGED, Interviewee Dr. Nakumara PHD, Tokyo Hospital senior staff.
EXPUNGED: Begin by explaining this so-called monster girl.
Dr. Nakumara: We don't know what she is exactly. She has pale skin, like pure white but not veiny, and a face devoid of any features save a gaping circular maw lined with sharp teeth like a lamprey. She can still see and hear things nearby somehow, perhaps eyes are behind that stringy black hair of hers? Other than the obvious she looks like any other girl around ten years old, when viewed from behind you wouldn't be mistaken thinking she was. She was naked initially but has since stolen dirty gowns from the laundry room to blend in with the patients. She even knows to abandon the clothes when they get too bloody.
Dr. Nakumara: Ah, yes, the problem with the one we've nicknamed "Teether"... she hunts human prey. She uses her small size and unassuming appearance as tools to lure in prey before ambushing them and tearing out their throat with her teeth. Afterwards she usually dessicates the body of all blood to feed. Any attempts to capture her alive haven't gone well, she posesses intelligence beyond that of a mere animal and has proven to be mallicious. She does speak, often taunting the security cameras and of course to lure in prey by pretending to be a scared little girl.
EXPUNGED: So there's an intelligence behind the attacks? Not just food motivation?
Dr. Nakumara: Of course, granted we also provoked a reaction with our attempts to prevent more bloodshed. That might sound too kind, but as a doctor I have sworn to do no harm. Can I ask that you try to resolve this peacefully?
EXPUNGED: We'll... see what we can do. That's all.

Tokyo Hospital has recently gotten a problem in the form of a girl-shaped entity with a lamprey mouth who feeds on human blood. Unusual Incidents Unit agents have already been dispatched to interview staff and assess the situation, they should have a blockade boxing in the entity known as "Teether". Isolate and capture her alive for study, but you are authorized to put her down if necessary. Alternatively if you aren't with the UIU... forget you read this. And stay out of the fourth floor!

The cold, dark section of hospital provided a creepy ambiance. The UIU had been quick to create a perimeter but slow to act, as per usual. The entity had breached containment once before you arrived and managed to kill a patient, but luckily it had been desperate enough after being starved from the isolation to leave a trail of blood. As you followed it down the hallway the loud echoing of your footsteps wasn't the only sound - somewhere up ahead you heard what sounded like a little girl sobbing.
As you followed the trail of blood into a former nurses' station you found it lead to the source of the noise, a little girl sat with her back to you. Her stringy black hair covered her face as she cried into her knees, the single functioning light shining on her from up above.

You knew better than to do so, but for some reason the fourth floor called to you. Those guys in suits talking funny said not to go there because it was dangerous. Regardless, the elevator dinged as the doors opened to a dark corridor with obvious signs of recent abandonment. Very panicked abandonment.
As soon as you stepped out of the elevator the doors shut behind you and you were plunged into darkness. The air conditioning had been left running which created a thick fog of freezing cold air that obscured visibility save for a few feet in any given direction. In the distance you could hear the echoing of a little girl's voice...?
