When he smiles, his mouth unnaturally widens, revealing fanged teeth. His sclera is pitch black, with two glowing red dots for irises and very small pupils. Notably, Exe always appears to have blood under his eyes, even when he's not crying. This blood is actually a significant birthmark. After creating his body, his eyes experienced intense pain, causing them to bleed. As a result, a faint but visible rash formed beneath his eyes from excessive bleeding.
When Sonic.exe is in his disguised form, he looks identical to the real Sonic. However, in his semi-disguised form, he begins to exhibit subtle changes that resemble a combination of his disguised and physical forms.
