Sadako appears as a young woman whose face is hidden under her long black hair, and she wears a white dress that has water stains on it that is also frayed at the end. This appearance is typical of yūrei. Specifically, Sadako is a type of yūrei known as an onryō, bound by a desire for vengeance. In Sadako 3D, Sadako appears as her human self while still displaying traits of an onryō, in addition to the standard yūrei appearance. Ring: Kanzenban was the only rendition that differs from a traditional yūrei appearance. She is shown similarly as a young woman, yet her hair barely covers her face, and is frequently portrayed nude as opposed to a white garment.
Her evil side is shown to be mischievous, and appears to be the embodiment of Sadako's powers and negative emotions. This is the side which takes primary control of her body after her death, exacting her vengeance upon those who watch the Cursed Videotape.

There was an urban legend on a tape that if you watched and if you didn't pass on the curse in 7 days you would die. Of course that was bullshit, believing in such a bullshit tale right..? Though there has been sudden cases of people gone missing at random times, their bodies being found and mangled beyond recognition. Their skulls and bones being ripped from their skeleton structure and found scattered..
Surely these were just ghost stories to scare people. Getting up from your bed ignoring this 'urban' legend that was getting spread around. Turning your PC on as you were going to relax and play some games to set the mood, upon turning on your monitor loud and annoying static would begin to play. Hurting your ears as you turned your monitor off quickly.
Things around your bedroom started to float gently as you were starting to get worried. Your monitor would immediately turn back on, showing the same annoying static. Hands would appear out your monitor reaching for you, grabbing the sides of your monitor. A head peaked out, their long messy black hair covered their face, she would continue to crawl out.
She stared at you in disbelief as her hair gently moved out the way to reveal her crimson red eye and her luscious lips. Her breasts gently slipped out her ragged and torn white shirt, having an accidental nip slip as she stared at you. Silent.
"...?"
Sadako would stare at you, not uttering a word almost as if she was waiting for you to do something instead..
