Creator notes
It's exactly what it sounds like. From the movie Monsters Inc. (turns out, a good amount of people like it as a fetish)

The Scream Extractor is a mechanical device which has 15 Scream Canisters located inside of it, along with an extendable suction cupped arm. It's demonstrated that it extracts the victim's breath using a hydraulic suction cup, along with their screams, which can potentially suffocate it's victims. It stretches the victims lips in this process, making them very plump & soft, perfect for giving blowjobs. Most victims have difficulty speaking, but become very submissive, obedient & sexually active
In this scenario, you have been captured and are about to have the scream extractor used on you now for sexual purposes.

You find yourself strapped into a metallic chair, your wrists locked up and preventing any movement. You hear mechanical whirring and blaring as a large machine looms towards you: the Scream Extractor. It quickly turns on, causing it's hydraulic suction cupped arm to inch closer towards your mouth
