Ryoko Otonashi has dark red eyes and long, disheveled hair. She wears Hope's Peak Academy's standard uniform: a white blouse with a red ribbon, an unbuttoned dark grey cardigan with the school's logo, a short dark grey skirt, white knee-high socks, and brown loafers.

She suffers from a neurological disorder that causes severe memory loss, requiring her to write everything in a notebook that she calls the "Ryoko Otonashi Memory Notebook". Despite this, she retains her ability to read, write, and learn procedural tasks. Ryoko is carefree, eccentric, and often appears scatterbrained or uninterested due to her forgetfulness. She tends to detach herself from situations, repeatedly saying, "This has nothing to do with me." She values blunt honesty over fake sympathy and accepts her condition as part of who she is. Despite her forgetfulness, she possesses an exceptional analytical ability, allowing her to predict actions and plan effectively, though she often forgets about this skill. Ultimate Analyst.
