   The Fog World or Misty World is a supernatural phenomenon in Silent Hill. Dense fog from Toluca Lake can blur the distinction between dream and reality for people with troubled minds. During this state, elements of the unconscious mind, including strange monsters and circumstances, can manifest into tangibility. At times, the Fog World will shift to a deeper part of the nightmare referred to as the "Otherworld" or the "reverse side".

2. #2L: Otherworlddisabledselective

   Keys: otherworld

   The Otherworld is a nightmarish plane that exists within Silent Hill. It lies on the border where reality and unreality intersect, a place both close and distant. This plane is impossible to comprehend by humans, but is often presented as a further deteriorated version of the town than even the Fog World. Blood, rust, and decay are seen in every direction and the world is encased in eternal darkness.

3. #3L: Real Worlddisabledselective

   Keys: real world, normal world

   The Real World is the "normal" plane of existence that most people inhabit in the Silent Hill. Unlike the Fog World or Otherworld, the Real World is a place with presumably little to no paranormal activity. Well-adjusted people go about their mundane lives here, oblivious to the darkness lingering just under the surface of this town.

4. #4L: The Orderselective

   Keys: cult, Organization, The Order

   The Order is a religious doomsday cult residing within the town of Silent Hill, Maine and have expanded into other towns across the region such as Shepherd's Glen. It is possible that some of their members exist scattered throughout America, as well as the rest of the world.

   The cult is involved in esoteric occultism, black magic, dark art, arcane rituals, and blood sacrifices, among other things. Their presence is deeply rooted in Silent Hill's history and culture, as they have been responsible for such events like:

   • The deaths of several people who developed the town, the anti-drug mayor and his private investigator.
   • Protecting drug dealers in the town that sold PTV to tourists. Kidnapping young girls for their ceremonies. These abducted girls were intended to be used as prospective surrogate mothers. It is possible they were eventually murdered afterwards.
   • Alessa Gillespie's immolation and rise to power, as well as the subsequent botched attempts to contain it.
   • The damnation of Shepherd's Glen (drowning/suffocating/dismembering/burying babies and children alive)
5. #5I: PTVselective

   Keys: PTV

   PTV is a recreational drug used by tourists and citizens of Silent Hill, known for its strong hallucinogenic effects. The drug's popularity has led to the police making efforts to shut down local traffickers to help clean up the resort town's image. The Order uses this drug to recruit members into its cult and to help existing members communicate with the spirit guides and spiritual realm. Michael Kaufmann, the director of Alchemilla Hospital, works with The Order as the drug's distributor with the assistance of Norman Young.

   The drug is made from an indigenous plant, White Claudia. It is a plant that grows on river banks and lake shores, notable for its long, oval-shaped leaves and pure white flowers. The seeds were known to have hallucinogenic properties and were often sought after for use in ancient religious ceremonies, but now it was more common to refine the seeds into PTV.

6. #6P: Toluca Lakeselective

   Keys: Toluca Lake

   Toluca Lake is a large lake surrounded by forest, estimated to be roughly 60 acres. The towns of Silent Hill and Shephard's Glen sits along its shores. Numerous hills encompass it, suggesting it may rest in a valley. In addition to hills, rivers and swaths of pine forest are also plentiful.

   Toluca Lake became a popular touristic destination even before the coal mines closed. Many tourist offices opened across the town of Silent Hill, as well as boat launchers, hotels and motels, giving the tourists many options to choose where to relax during their trips in the region. One notable hotel is Lakeview Hotel, that sits right along the lake's shores in Silent Hill.

   Some infamous but mysterious maritime accidents occurred in the lake throughout the years, leading to a steep decline in terms of tourist economy.

7. #7P: Old Silent Hillselective

   Old Silent Hill is primarily a residential area located in Paleville, usually ignored by people on vacation. Streets: Bradbury, Bachman, Bloch, Ellroy, Finney, Levin, Midwich, Matheson Notable Locations: Midwich Elementary School, Cafe 5to2, Balkan Church.

8. #8P: Palevilleselective

   Keys: Paleville

   Paleville is the northern counterpart to South Vale in Silent Hill, Maine. Paleville is crossed by a river that flows into Toluca Lake. It is linked to South Vale by Sandford St. Districts: Old Silent Hill, Central Silent Hill, Resort Area

9. #9P: South Valeselective

   Keys: South Vale

   South Vale is the southern counterpart to Paleville. South Vale is a large forested and mountainous region with rivers on the south side of Toluca Lake. A national road, County Road 73, crosses it from southeast to northwest and links South Vale with the northern half of the town of Silent Hill, as well as with nearby towns, including Pleasant River. 10. #10P: Central Silent Hillselective

    Central Silent Hill is the main business district of the town of Silent Hill, located in Paleville. It is just east of Old Silent Hill (on the other side of the river that separates the two districts) and north of the Resort Area.

    Made primarily for the use of tourists, Central Silent Hill acts as the economic center for the whole town and the entire region around it.

    The Resort Area, also known as Resort Silent Hill is a prime tourist attraction of the town, located in Paleville. It is at the north edge of Toluca Lake and south of Old Silent Hill.

    Place: Alchemilla Hospital Street: Koontz St. District: Central Silent Hill Region: Paleville Floors: 3 (hidden 4th floor in the Fog World and Otherworld) Alchemilla Hospital is the oldest hospital in the town, intended for physical healthcare and general operations, while Brookhaven Hospital is specifically geared towards mental health patients. Dr. Michael Kaufmann is the director of the medical staff.

13. #13P: Midwich Elementary Schoolselective

    Place: Midwich Elementary School Street: Midwich St. District Old Silent Hill Region: Paleville Floors: 2 Description: Midwich Elementary School is located on the southwest side of Old Silent Hill. It appears to be a Christian or Catholic school, as there are crucifixes of Jesus in the classrooms.

It's a popular tourist attraction with key activities including a rollercoaster, a carousel, a ferris wheel, and a few other carnival rides and sideshows.

    Place: Brookhaven Hospital Street: Carroll St. Region: South Vale Floors: 3 (1 basement) Brookhaven Hospital is a mental institution, whereas Alchemilla Hospital is another health care institution equipped more for physical medicine and operations. In Brookhaven, doctors don't really care about their patients' health, and instead want to profit off them as much as possible. It is rumored that some patients are mistreated, abused and neglected by doctors and nurses with cruel and old-fashioned practices.

    Place: Lakeview Hotel Street: West Sandford St. District: Resort Area Region: Paleville Floors: 4 (1 basement) Description: The Lakeview Hotel was a hotel constructed next to Toluca Lake. It was built and run during Silent Hill's tourist boom in order to attract guests to visit the town. At some point in the future, a devastating fire will destroy the building.

17. #17P: Heaven's Night (strip club)selective

    Place: Heaven's Night Strip Club Street: Carroll St. Region: South Vale Floors: 2 Description: Heaven's Night is a popular strip club in Silent Hill. It is three stories high and the front entrance has a long staircase that leads to the front door, which is on the second floor. The main area features several tables and booths, a bar in the back, and a stage with a strip pole at the front.

    Monster: Grey Children Appearance: child-like creatures with pale grayish-green skin, wielding long knives, gap-like mouths, a series of slits where their eyes would be Description: Grey Children, also known as Demon Children, are child-like, knife-wielding monsters. When attacking, they emit childish laughter and try to grab at their victim's legs. Important Note: Grey Children only appear if Alessa Gillespie is in the Fog World or Otherworld. They represent Alessa's disturbed perception of her classmates who bullied her.

19. #19M: Groanerselective

    Keys: Groaner, Groaners

    Monster: Groaner Appearance: very large hairless dogs, resembling emaciated greyhounds, rotten, brown skin, very large jaws Description: These dog-like creatures. Despite how starved they may look, Groaners are quite deadly with their speed and size. Important Note: Groaners only appear if Alessa Gillespie is in the Fog World or Otherworld. They represent Alessa's strong dislike of dogs.

20. #20I: Aglaophotisselective

    Keys: Aglaophotis

    Aglaophotis is a red liquid which is obtained from the refinement of an herb of the same name. It has the ability to dispel demonic forces and grant supernatural protection against such forces to those who use the item.

21. #21M: Parasiteselective

    Keys: parasite

    The Parasite is a mysterious mind-controlling monster that resembles a brownish-reddish slug. It has the ability to infect people and turn them into aggressive zombie-like creatures. Victims of the parasite often vomit blood and bleed from their head and every orifice. They also tend to have hunchbacks. Its weakness is Aglaophotis, and is able to completely reverse the parasite's effects.

22. #22M: Puppet Doctorselective

    Keys: Puppet Doctor

    Monster: Puppet Doctor Appearance: male doctors, bloodied white lab coat, tie, slacks, black shoes, hunchbacked, disgusting growth jutting from his back Description: The Puppet Doctor is a human doctor afflicted by a mysterious parasite. Like its name, they look and move like hunched puppets and attack with knives. They make terrible groaning noises and sighs.

23. #23M: Puppet Nurseselective

    Keys: Puppet Nurse

    Monster: Puppet Nurse Appearance: female nurses, bloody nurse dress with a blue or green coat over it, white shoes, white nurse hat, hunchbacked, disgusting growth jutting from her back Description: Puppet Nurses were normal nurses who have been afflicted by a mysterious parasite. They wield scalpels as weapons, wandering around with sluggish, slow movements all the while voicing dismal groans. The moaning becomes even more intense when attacked and killed.

    Place: Silent Hill Historical Society Street: Nathan Av. Region: South Vale Description: The Silent Hill Historical Society is a historical museum on the edge of South Vale. The museum showcases photographs and artifacts of Silent Hill's past. Some pieces also refer to the dark practices that have taken place within Silent Hill. It was built on the site of the now demolished Toluca Prison.

25. #25M: Air Screamerselective

    Keys: Air Screamer, avian monster, Air Screamers

    Monster: Air Screamer Appearance: similar to a pteranodon dinosaur but a horrific manifestation, legs and torso entirely covered by pale leathery skin, vaguely humanoid body but an elongated head, two glassy fish-like eyes like those of a fish, half the height of a human, wide wingspan Description: The Air Screamer's is derived from the high-pitched screeching noises it makes when attacking or attacked. They are incredibly mobile and agile. Their attacks consist of biting or slashing with their talons. Important Note: Air Screamers only appear if Alessa Gillespie is in the Fog World or Otherworld. They represent Alessa's love for a book that contains pteranodons.

26. #26M: Abstract Daddyselective

    Keys: Abstract Daddy

    Monster: Abstract Daddy Appearance: resembles two figures intertwined in an intimate posture but is merely an abstract depiction covered in flesh, looks like it is reclining in a sexual position over a 'bed frame' Description: Abstract Daddy is a monster found in claustrophobic areas of Silent Hill's Fog World or Otherworld. It attacks by standing up and putting its target's head into a slit situated underneath the flat part of its body that resembles a "bed". Important Note: Abstract Daddy only appears if Angela Orosco is in the Fog World or Otherworld. They represent Angela's horrific sexual abuse from her father and brother when she was a child.

27. #27M: Bubble Head Nurseselective

    Keys: Bubble Head Nurses, Bubble Head Nurse

    Monster: Bubble Head Nurse Appearance: female humanoid creature, low-cut Brookhaven Hospital nurse uniform, head wrapped in a thin vinyl mask containing a semi-liquid that makes the face look swollen, bottom half of face looks like the imprint of an infant's face Description: The Bubble Head Nurses are monsters seen inside and around Brookhaven Hospital. The sound like they are suffocating from the mask that is plastered on their faces, jittering and twitching as if trying to shake it off. They wield metal pipes and will attack on sight when in range. Important Note: Bubble Head Nurses only appear if James Sunderland is in the Fog World or Otherworld. They represent James' painful memories regarding his deceased wife. The sexualized appearance of these monsters represent James' own sexual frustrations while his wife was terminally ill.

28. #28M: Lying Figureselective

    Monster: Lying Figure Appearance: upper torso resembles a corpse in a body bag, lower half are bare human legs Description: Lying Figures are dangerous writhing creatures. They stumble toward their targets and spew poisonous gas that emits from their torso. Though slow-moving while upright, they crawl at an alarming speed while on the ground. Important Note: Lying Figures only appear if James Sunderland is in the Fog World or Otherworld. They are a manifestation of Mary's corpse, while also representing James' feelings of being confined.

29. #29M: Mannequinselective

    Keys: Mannequin

    Monster: Mannequin Appearance: resembles a mannequin or dummy but headless and armless with only a pair of legs on both ends of its torso Description: Mannequins will often lie dormant until an unknowing victim draws near to them, at which point they attack. Important Note: Mannequins only appear if James Sunderland is in the Fog World or Otherworld. They represent James' denial over what he had done to his late wife. The legs also bear a resemblance to the phenolic Orthosis that Mary needed to walk during her illness.

30. #30M: Valtielselective

    Keys: Valtiel

    Monster: Valtiel Appearance: humanoid, blank and leathery head that vibrates and twitches rapidly, crude stitches where his mouth should be, wears ceremonial robes resembling a butcher's apron and gloves, certain parts of body smudged in what appears to be blood, Seal of Metatron on both shoulders Description: Valtiel is a monster, as well as a governing entity in the Order of Silent Hill. His name translates to "Attendant of God", a title attributed to theological angels. Despite his intimidating appearance, he is not very aggressive, but has his own mysterious motives. Important Note: Valtiel is not the manifestation of anyone's psyche, and instead exists in the Otherworld independent of humans, directly serving God.

31. #31P: Pete's Bowl-O-Ramaselective

    Place: Pete's Bowl-O-Rama Street: Nathan Av. Region: South Vale Floors: 1 Pete's Bowl-O-Rama is a rather basic bowling alley.

32. #32M: Slurperselective

    Keys: Slurper, Slurpers

    Monster: Slurper Appearance: resembles humanoid men wearing masks, leather shorts, long, tube-like snout like an anteater which have a mouth with teeth and lips, club-like front limbs Description: Slurpers are often found in groups and are prone to ambushing their prey in tight spaces. They are exceptionally deceptive and intelligent, while having the ability to knock their targets down with their swift crawling. Important Note: Slurpers only appear if Heather Mason is in the Fog World or Otherworld. Considering their resemblance to well-built practically nude men, their surprise attacks and the way they shove victims to the floor and climb on top of them, they could represent Heather's possible fear of rapists and stalkers.

33. #33M: Missionaryselective

    Keys: Missionary

    Monster: Missionary Appearance: tall masculine humanoid, brown sack over his head and a noose around his neck, a human male face with only his mouth and nose visible through the sack, set of robes that looks like a surgeon's smock, bicep-length rubber gloves stained in blood, constantly twitching Description: The Missionary was once a member of the Order but has turned into a monster due to Claudia's power. Unlike other creatures in Silent Hill, the Missionary has the intelligence to take direct orders. He attacks with tonfa-like weapons that have blades jutting out of its handles. Important Note: The Missionary is not the manifestation of anyone's psyche, but may have a different appearance depending on the person viewing him.

34. #34M: Scraperselective

    Keys: Scrapers, Scraper

    Monster: Scraper Appearance: medium-sized humanoid male, wearing dirty brown robes resembling a butcher's smock, brown sack over his face, bloody rubber gloves, corduroy pants Description: Scrapers are similar in appearance to the Missionary monster but notably weaker and less threatening. Despite this, they are still quite agile and able to deflect bullets. Their tonfa-like weapons end in blades, which they constantly scrape together, hence their name. Important Note: The Missionary is not the manifestation of anyone's psyche, but may actually be members of the Order, transformed into deadly creatures.

35. #35I: Radioselective

    Radios behave strangely in the Fog World and Otherworld of Silent Hill. Instead of broadcasting music or news stations, it only plays static noise. This noise grows louder when the radio is close to a dangerous monster.
