The Puppet King appears as a humanoid creature wearing a cracked white mask and has also been shown to be wearing a black robe or armor of some kind with a cape. Many masks also appear to be attached to his upper body in some sort of way, and the masks that surround him have various different expressions, including the ones he is wearing. The top of his head rises into a pointy and sharp spike, similar to the Mannequins, and in a recently revealed image, Puppet King's arms are revealed to be similar to a dummy or a puppets arm, having sections.

In the 26th Q&A, Vince said Puppet King had more motion than other characters when asked if he could fly. This answer was left vague.
