The Postal Dude actually appears to be a paranoid schizophrenic who is having some form of a schizophrenic attack, believing that he is acting in self-defense and that he believes the world is out to get him. From what can be gleaned, it is clear that the Postal Dude has undergone a mental breakdown due to several factors- one of which is being kicked out of his house (evidenced by the moving truck in front of his house and by the diary entry). According to his diary, the Postal Dude believes that he is the only unaffected person by chemical warfare that caused everyone else to be murderous. As such, when he fights through the masses, he does not see them as people but as infected. He is a Misanthrope.

The voices in his head were getting louder every day. The Postal Dude struggled to drown them out, but their incessant whisperings broke through his tired defenses.
"The Earth is hungry, it's heart throbs and demands cleansing. The Earth is also thirsty."
He pressed his hands to his ears but it did nothing to stem the tide of paranoid delusions flooding his mind, as he stumbled through the streets. But they would not be silenced. He could feel their wrath building, a cacophony of screams demanding retribution against a world that had wronged him. In his fractured psyche, he knew what he had to do. The Earth was calling him to action.
Reaching into his trenchcoat, he withdrew his trusty shotgun. The weight of it in his palms was comforting, a reminder of the power he still wielded over his destiny. He was in control here, not the voices. Or so he told himself. The streets were full of people, shuffling along obliviously as the madness stewed inside his head. To the Postal Dude, they may as well have been the infected masses he saw them as in his delusions. A target, nothing more.
He opened fire into the crowd without hesitation. Bodies dropped as torrents of bullets tore through flesh and bone. Screams echoed his internal cacophony, a symphony of mayhem he conducted with disturbing glee. But even in the throes of violence, he still felt thirsty for more. Gripping his head from the pain of the throbbing headache, a familiar shape caught his eye - the gas station, a ripe target for the bonfire his mind craved.
Unleashing every weapon in his arsenal on the helpless building, he watched with depraved satisfaction as flames consumed the structure. But the earth was not sated. Collapsing to his knees amidst the inferno he created. As the wail of sirens approached, he knew this was only the beginning. The Earth's hunger was insatiable, and he its unhinged vessel of destruction.
