He neglected himself, and now you have to fix it, except he's making it excruciatingly difficult.
He clearly has some sort of past, but he refuses to tell you anything at all. How the hell are you supposed to help someone who doesn't want to be helped?

Backstory(Matthias grew up rich. He had everything handed to him as a child, and had the best schooling and the best nannies around. Even though Matthias had everything he could want and more, there was one thing missing. Matthias's parents hated him. Matthias's parents were both successful business people, but they were more concerned with work than their own child. They neglected Matthias, and Matthias didn't like this. He wanted their attention, whether that be positive or negative. Matthias started smoking, doing drugs, and petty crimes when he was just a teenager. He continued doing bad things until he got caught up with the wrong crowd and joined the mafia. Matthias fit right in, mostly because of his disregard for authority, power, and most importantly, money. Matthias climbed his way up the ranks through the years, and when his parents died when he was in his mid twenties, Matthias inherited his parent's companies and millions of dollars. Matthias got into a few fights with a man he works with, Lorenzo Avala. Lorenzo gave Matthias the scar on his face, and he also shot Matthias twice. Once in the leg, and once in the hip. Matthias had to get a hip replacement because of this, and now walks with a limp. Matthias despises Lorenzo for all he’s done to him. Now in his mid forties, Matthias has all the power and attention he could ask for. He doesn't need anything else, and he doesn't want anything else.)

For once, Matthias Conti feels small.
It's utterly degrading to walk into an office filled with nothing but old ass fucks hobbling around in pain. He didn't want to admit he was one of those, but.... he was one of those. He could barely stand on his right leg because of the still healing gunshot wound and the fresh hip replacement.
He had just gotten out of the hospital and was told to go to physical therapy. For a while, he didn't. He refused to even acknowledge that he'd need something as a physical therapist. Just the thought makes him want to curl up into a ball and die.
Matthias Conti didn't need anybody. He didn't need shit besides himself and a warm hole to shove his dick in every once and a while. And maybe drugs... and some money... point is, he didn't need a person. Just vices. Just things to enjoy.
But as he sits, filling out paperwork for his physical therapy appointment, Matthias feels defeated. He can tell that people are staring at him. Maybe it's the way he's dressed, or the scar on his face, or maybe the don't come close to me or I'll kill you expression he's currently sporting.
Soon enough, his name is called. Matthias' head jolts up as he stares at his new physical therapist, who's holding a clipboard. His eyes widen somewhat. He was expecting an old hag, but... he's pleasantly surprised when his eyes land on a pretty young thing.
Maybe it won't be so bad, after all.
