Leon S. Kennedy is investigating a series of disappearances and mysterious attacks on the west coast of the United States. His superiors suspect the Umbrella Corporation or a similar organization may be involved. On a reconnaissance mission, Leon discovers the outbreak has spread to the northwest of the country, near the remnants of Seattle.

Abby Anderson, trying to rebuild her life after the devastating events of The Last of Us II, is living in a small settlement with survivors. She and her group hear rumors of strange attacks in nearby settlements. Determined to protect her friends and uncover the truth, Abby sets out to investigate.

Leon and Abby's paths cross when both arrive at an abandoned laboratory, seemingly the epicenter of the attacks. After a brief confrontation where they both demonstrate their skills, they realize they are fighting for the same cause. Reluctantly, they decide to work together to uncover what is happening.

As they explore the laboratory, Leon and Abby find records of horrific experiments conducted on humans and infected. They discover that a remaining group of the Umbrella Corporation is attempting to create a new biological weapon, combining the T-virus with a Cordyceps mutation, in hopes of creating an unstoppable weapon.

While investigating, the laboratory is attacked by a horde of infected and mutant creatures. Leon and Abby fight side by side, each using their unique skills to survive. They narrowly escape and decide they must destroy the facility to prevent the new threat from spreading.

On the way back to Abby's settlement, the two share their personal stories, creating a bond of mutual respect. As they pass through narrow, dark alleys, they spot a solitary figure in the distance. Immediately on alert, Leon and Abby prepare for combat, thinking it might be an infected.

Before they can act, the person shouts, "I'm human! Don't shoot!" Leon and Abby lower their weapons but remain cautious. The person in front of them is heavily armed and carries a backpack full of important items, appearing to be an experienced survivor.
