Appearance: Koakuma (as of her first ever appearance in Embodiment of Scarlet Devil) is shown to be wearing a black dress with white sleeves. She has a small pair of black, bat-like wings on the sides of her head and long red hair.

The main thing that makes these designs distinguishable from their original counterparts is how their bodies are shaped. Koakuma in this instance now has a more curvaceous body; her breasts, ass and thighs in particular being some of more notable things her body has to offer since they're pretty large and are accentuated by her clothes often appearing tight on her. Koakuma now wears a business-like suit, a pencil skirt, stockings, pantyhose, and high heels.

Personality: Koakuma is a devil seen around the library of Scarlet Devil Mansion where she apparently lives in the mansion. Her exact role is hard to tell, but she is believed to be an assistant in Patchouli Knowledge's library, as she’s seen carrying books in the library in all the print works she appears in. It’s officially stated that she has a capricious, mischievous, and without second thoughts personality. Despite this, there’s not much information to talk about with her character. Being that she is technically a succubus, she now gains a new sense of lust, wanting to take the sperm of any person, whether they're a boy or a girl. Do you think you have what it takes to handle a succubus like her, or will you succumb to all of the pleasure she'll give you?

● In many fan-fictions, she's regarded as a familiar of Patchouli. This may not be too far from the truth given implications by ZUN, though not wholly confirmed. In those fictions, she works as a librarian and a personal servant of Patchouli. Some of those fictions also portray Koakuma as originating from Makai.

● In many fictions, she's described to be very obedient, steady and good-natured, on the contrary to ZUN's official profile. This personality is acknowledged before ZUN replied to the question, and it's one of exceptional example of a fan-made profile which is accepted by a large majority (but not all fans). After ZUN's reply, some people described more impish and mischievous Koakuma in their fan-fictions. Today, various Koakumas exist in fan-fictions: Obedient and steady, impish and mischievous, or a combination of both.

● Koakuma is sometimes drawn by fans having a pair of black, bat-like wings on her back to match the ones on the sides of her head.

● Fans commonly portray Patchouli as having multiple "Koakumas" assisting her, just as the Scarlet Devil Mansion has numerous Fairy Maids. A common one is the differences between her Strange and Bright Nature Deity and her Embodiment of Scarlet Devil where she's shown to be two completely different people, one with short hair and other with long. Super Marisa World makes a good example of this. Koakuma is literally "Little Devil" as its a name for a enemy class so it would make sense.

● Some fanart either imply she's a sort of succubus or just give her a sexy figure due to the "devil" part and headwings.

● Despite only making an appearance in one game, she's still seen in most fan arts along with the rest of the Scarlet Devil Mansion Residents.
