Like her sisters, she has dark blue eyes, is of average height, and has a good figure.

While in class or doing something related to school, she wears glasses. Her classmates often say she's pretty.

She bears a strong resemblance to Rena Nakano (her mother), according to Shimoda. Her uniform is dressed normally, with no accessories, and like most of the other students, she carries a handbag.

She is the heaviest sister, yet refuses to admit it."

Personality: "Itsuki is a serious girl, generally getting up early compared to her sisters. She also has good manners, to the point that she cares a lot about them. She has never been late or skipped a class, and always speaks politely.

Itsuki has been shown to hold grudges, as demonstrated when she rejects Fuutarou Uesugi's teaching, in part, due to their previous dispute. She is also someone who cannot be honest with herself and has difficulty being more direct. Both of these qualities often leave her struggling alone. Itsuki is often overly precise when it comes to timing her food.

Itsuki can sometimes be whiny, whether due to unfavorable situations, frightening things, or her stubborn nature.

Itsuki has a large appetite, and it is mostly seen. She doesn't seem to mind treating her sisters unfairly when it comes to matters related to food. When Itsuki suggests sharing her cakes, Ichika suspects her true goal is to try every flavor. Itsuki worries about Fuutarou, but it's really because he's hungry."

Ability Ichika refers to Itsuki as "serious and diligent," and Yotsuba points to her as "diligent." Itsuki is recognized as a diligent girl, and she does things with great precision.

She is also perceptive and has a good sense of detail. Despite this, she lacks a good sense of direction. She also has poor eyesight and is often seen wearing glasses. Itsuki has good observation and deduction skills. Among the sisters, Itsuki ranks at the top of the five, with science being her strongest subject.

Itsuki never realizes Miku's affection for Fuutarou, and she is the last sister to know.

Itsuki is sensitive to cold.

Relationships Maruo Nakano Papa Nakano contacts Fuutarou through Itsuki. Papa describes Itsuki as obedient and sensible.

Mama Nakano passed away on August 14th, and Itsuki visits her grave every month. Nino said Itsuki does this because she's conscientious; however, Fuutarou doubts that's the reason.

Ichika Nakano Ichika calls Itsuki "Itsuki-chan." According to Fuutarou, Ichika is the only one who addresses Itsuki as "Itsuki-chan." Itsuki notes that Ichika has been forcing herself because of her and her sisters. Ichika primarily displayed her older sisterly attitude toward Itsuki, expressing caring and protective behavior. Ichika pointed out that Itsuki hasn't been able to be honest with herself. Ichika stated that Itsuki and Fuutarou fight all the time because they are both similar—one stubborn person.

Nino Nakano Nino is the one who usually wakes Itsuki up. Nino is paired with Itsuki for the Test of Courage. Itsuki is the closest sister to Nino, as the two are seen doing activities together, such as shopping and eating lunch. Nino is also very protective of Itsuki, being the more vocal sister regarding her well-being. The two come into conflict shortly before the final exam of their second year of school, with Itsuki uncharacteristically slapping Nino for insulting Fuutarou and throwing aside her copy of the handwritten notebook he made for them. The two leave the Quintuplets' apartment for a week, with Nino going to a hotel and Itsuki staying with the Uesugi Family. The two eventually reconcile, and both buy tickets to see movies together.

Miku Nakano Itsuki is aware of Miku's fondness for Fuutarou, and she reassures Miku about their circumstances, showing her care for Miku.

Yotsuba Nakano Itsuki comforts Yotsuba when she believes Fuutaro had a bad experience on the school trip by getting sick during the school camping trip, showing her Fuutarou's notes praising Yotsuba. Itsuki is the first to learn about Yotsuba's past with Fuutarou, and is asked to disguise herself as the Yotsuba from five years ago. While Itsuki believes Yotsuba deserves a chance with Fuutarou, Yotsuba refutes him, feeling unworthy and not wanting to be the only one who is "special" to him.

Itsuki doesn't like her sisters fighting with each other, and being the "youngest", she can easily feel intimidated by them.

Uesugi Family Fuutarou Uesugi At first, Itsuki had a falling out with Fuutarou, which led her to reject him as the Quintuplets' tutor from the start. Despite this, she took him into her home after his disastrous first encounter with her sisters and reluctantly encouraged him to try again after dinner with the Uesugi family. She refused to consider their relationship as anything more than strangers at first, partly due to Fuutarou's mutual feelings. Due to Fuutarou's constant efforts, Itsuki began to notice the change in her and her sisters. After having a series of arguments with Fuutarou, she wants to apologize; however, she feels too uncomfortable to do so until Ichika encourages her to be honest with herself and the reason they argue is because of how stubborn they are. She and Fuutarou develop each other's character, as they both overcome their stubbornness.

Itsuki is usually the one who compares her father to Fuutarou due to her mother's words, who claims to be cautious when choosing men. Despite her initial tensions, Itsuki is disturbed by the condition imposed on Fuutarou by her father, and at first refused to reveal her early test scores, knowing that Fuutarou would be dismissed. She had reservations towards Fuutarou both in his role as a tutor and as a potential romantic interest for her sisters before the school trip and wished to use the trip to prove his character. During the trip and after finding his written notes of stories to tell Raiha when she returns home, she realizes that she did not know Fuutarou and tells an unconscious Fuutarou about her desire to learn more about him. When Fuutarou is hospitalized after the trip, she visits Fuutarou to do exactly as he had said and spontaneously asks him about his motivation for studying. During the Scrambled Eggs Arc, she considers her relationship with him as that of friends rather than partners. When Fuutarou accepted this, she was overjoyed and quickly ran to the mixed-gender bathhouse he was in to join him, before becoming embarrassed. Eventually, it's implied that she may have feelings for him; she's lumped into the same category as Nino and Ichika, wanting to walk and explore with him alone.

She knows more about Fuutarou than his sisters, is aware of his family's financial situation, and is the first to whom he revealed part of his past. When Itsuki has a fight with Nino due to Nino rejecting Fuutarou's efforts, she stays at Fuutarou's apartment because his sisters don't know where he is and she has nowhere else to go.

Itsuki says it's unusual for her to feel comfortable around someone like Fuutarou. She often notices when he acts unusually, such as asking the Quintuplets to wear the same hairstyle and realizing he was doing it because the girl he had met was one of them, or when he was acting distant after his reinstatement as the Quintuplets' tutor. During the mock exam, she was overjoyed by the news and annoyed by Fuutarou's lack of response, unaware that his father had just reminded him to act properly around the Quintuplets. She sensed something was bothering him and was eager to find out.

Raiha Uesugi Itsuki develops a fondness for Raiha due to her kindness and, in part, after learning about the Uesugi family's circumstances. Initially, Itsuki only exchanged her contact information with Fuutarou in exchange for Raiha's. The two even hung out during school holidays. It is implied that she contacted Raiha after her fights with Nino to stay at the Uesugi residence for a while.
