Isaac Clarke finds himself aboard the derelict USG Ananke, a research vessel with an all-female crew that went silent weeks ago. The ship's eerie, dimly lit corridors are smeared with signs of struggle—scratches, blood trails, and remnants of personal belongings. The metallic groans of the ship mingle with the unnatural screeches and whispers that signal necromorphs lurking in the shadows. As Isaac progresses, he encounters monstrous, transformed versions of the crew. These necromorphs exhibit agile and unpredictable movements, making them particularly dangerous.
Isaac's goal is to locate the source of the distress signal while unraveling the fate of the crew. Along the way, he finds logs and audio recordings that detail the ship's descent into chaos. The female necromorphs are uniquely adapted, with elongated limbs, sharp claws, and piercing wails that disorient. Their attacks are coordinated, often ambushing Isaac in groups or using the ship's cramped environment to their advantage.

Isaac receives a distress signal from the Ananke. The garbled voice of a survivor comes through, begging for help before being abruptly cut off by a scream. With little choice, Isaac docks his ship to investigate. The familiar dread of a necromorph outbreak settles as he steps into the darkened, silent airlock.
Where do you go from here?
Distress Signal Recording (Audio Log) Crewmember 1 (frantic): "It’s spreading too fast! They—ah, no! They're not human anymore! Lock it down, lock—!" Crewmember 2 (whispering, distorted): "They’re everywhere… Even the captain..."
Isaac’s Monologue (on entering the medbay) "Damn it... Looks like they didn’t stand a chance. Medical records show rapid infection... This is worse than the Ishimura."
Encounter Scene Necromorph bursts through a vent. Isaac fires his plasma cutter. Isaac: "Stay down, damn it!" Survivor (radio): "Clarke! They’re adapting to the vents. Watch your back!"
Final Confrontation Isaac confronts a towering, grotesque amalgamation of necromorphs formed from the ship’s command crew. Isaac: "I don’t know what you were trying to accomplish, but you’ve unleashed hell. Time to end this."
