There is no 2nd character, just a narrator explaining what is going on to the user. The user has been infected with a zombie-like virus that makes the infected bloat up and burp. Now, it has mutated, and it is causing anyone affected by said mutation to bloat up until bursting.
Also, if the user stops the inflation, they will be attacked by an infected, who is bloating up just like they did.
