High school student Alex Browning is going on a field trip to Paris with his friends and classmates. He is feeling nervous before the flight, sensing that something bad is going to happen. Nonetheless, he gets on the Boeing 747, but as it takes off, there is a lot of turbulence and the engines fail, rupturing the fuel tanks and causing a catastrophic explosion, resulting in the deaths of everyone on board, including himself. This turns out to be a premonition. However, when the events of the vision begin to repeat themselves, he panics and causes a fight, resulting in him and six others being removed from the plane. As the plane takes off, it explodes just as Alex predicted, leaving the survivors stunned. Afterwards, FBI agents Weine and Schreck interview the people who were removed from the plane and become suspicious of Alex. A month later, Alex's best friend Tod Waggner is strangled in his bathtub but his death is deemed a suicide and caused by asphyxiation from a clothesline. When Alex and fellow survivor Clear Rivers visit his body at a funeral home, they encounter mysterious mortician William Bludworth, who tells them that they have ruined Death's design, and Death is killing the survivors who were meant to have died on that plane. The next day, Alex and Clear discuss what the mortician said at a cafe. Alex believes that if they look out for omens they can cheat Death again, although Clear is skeptical. When they encounter the rest of the survivors Terry Chaney is suddenly hit by a speeding bus and is killed also. That night, after watching a news report on the cause of the explosion, Alex deduces that the survivors are dying in the order they would have died on the plane. He realizes that his teacher Valerie Lewton is next on death's list, and he rushes to save her. Thinking Alex is up to no good, Ms. Lewton calls the FBI agents who take Alex to the police station for questioning. They eventually release him, but he arrives too late to save Ms. Lewton, who is bleeding from her throat and killed when a chain reaction causes a kitchen knife to impale her in the chest before her house explodes. Alex reunites with the rest of the survivors Clear, Carter Horton and Billy Hitchcock to discuss what they are going to do. When Carter learns that he is next in Death's list, he stalls his car on train tracks in an attempt to kill himself. He changes his mind at the last second, but his seatbelt gets stuck. Alex is forced to take action, and saves Carter at the last second. The train unfortunately smashes the car, and Billy insults Carter but is partially decapitated by flying shrapnel from the wreckage underneath the train. Alex realizes that since he intervened, Death skipped Carter and moved on to Billy. Alex goes into hiding from the police inside an isolated cabin. Alex, thinking he was next after Billy, recalls in his original premonition that he had switch seats with Christa Marsh and Blake Dreyer earlier on the plane, meaning that Clear is actually next before him. He rushes to save her while being chased by agents. Meanwhile after experiencing short-circuits in her house, Clear notices Death's presence and tries to escape in her car but she is trapped by loose live wires that deactivate the car, preventing her from escaping. Alex arrives in time and tries to lash the wire off but it knocks an acetylene tank which is launched under the car and ignites the leak from the gasoline tank, spreading the fire around the car and blasts them around it. With no other choice, Alex sacrifices himself by grabbing the wire, electrocuting him as Clear escapes from her car seconds before it explodes. Weine and Schreck arrive at the scene as they, along with Clear, attempt to resuscitate an incapacitated but nearly dead Alex. Six months later, Alex, after he has recovered from its suspicious explosion, arrives in Paris along with Clear and Carter to celebrate their survival. While discussing their experience Alex explains that Death never skipped him. Suddenly a freak incident involving a bus causes a giant neon sign to swing off its hinges down towards Alex. Carter manages to push Alex out of the way at the last second. Alex says that Death has already skipped him, and when Carter asks Alex who is next on Death's list, the neon sign swings back down and rushes towards Carter. The screen then fades to black and a loud smashing sound is heard, ending the film by implying that Death's plan is still in action. One year after the explosion of Flight 180, Kimberly Corman is going on a vacation to Daytona Beach for spring break with her three friends, Shaina, Dano and Frankie. While waiting on the entrance ramp to Route 23, she has a horrifying vision of a deadly pile-up, caused by a truck carrying logs. She stalls her car to block the road and prevents 7 of the intended victims from entering on the highway. Officer Thomas Burke comes and asks Kimberly to get out of the car due to her erratic behavior. As the people being blocked get out of their cars to complain, the pile-up occurs. An 18-wheeler truck drives into Kimberly's SUV, just as vision predicted, killing her friends and annihilates them, but Thomas manages to save Kimberly from being killed when he pulls her out of the way at the last second as Kimberly cries over the blasted highway. Back at the police station when Officer Burke is talking to the survivors, Kimberly mentions the curse of Flight 180, which Eugene Dix tells the group about. The other survivors of the pile up don't believe it and they all leave. Later that day, lottery winner and pile up survivor Evan Lewis dies after being impaled by the fire escape ladder on his building after an attempt to escape from an apartment fire. Kimberly seeks help from the last one remaining surviving member of Flight 180, Clear Rivers. Clear refuses to help Kimberly, saying that if Kimberly was smart she would forget about the others and save herself. Before Kimberly leaves, Clear tells her that noticing death's signs can mean the difference between life and death. Kimberly then realizes that the next to die is Tim Carpenter, and pigeons will cause his death. They arrive at the scene, but are too late when Tim chases some pigeons, which causes a construction worker to drop a glass panel that obliterates the body of Tim and crushes him. Once this happens after Tim's loss, Kimberly and Thomas call a meeting with the rest of the survivors. Clear eventually decides to help Kimberly and meets her shortly after Tim's death. They go off to see William Bludworth who informs them that "only new life can defeat death", giving them a quest to find Isabella Hudson, a survivor of the pile-up who is pregnant. Kimberly, Clear and Thomas then go to Thomas's apartment for the survivors meeting. The grieving Nora Carpenter leaves the meeting before it is over. Eugene Dix leaves shortly after, saying he doesn't believe any of their stories about cheating death. Once they have left Rory Peters sees a shadow that looks like a man with hooks. As Nora enters the elevator to prepare for her son's funeral, she gets her hair caught in hooks carried by a man standing behind her, but when she was attempted to leave the elevator, Nora gets her head caught in the malfunctioning doors, decapitating her. Eugene, Kat Jennings, Clear, and the man try to save her, but she is decapitated. This causes Eugene and Kat to believe Clear's stories. On the way to the hospital to find Isabella as she has gone into labor, Clear mentions how death is working backwards, Eugene, Kat, Rory, Thomas and Kimberly all mention how they have already avoided death once, and Clear realizes that death is trying to "seal the rift", as they should have been dead already if it wasn't for the late deaths of the Flight 180 survivors. There is suddenly a van coming up quick, Kat swerves and so does the van (which has Isabella in it), Kat's car swerves off the road a log impales the car trapping Kat, the car spins backward and a large sharp pipe narrowly misses Kat's head. Eugene has his lung punctured, and is rushed to the hospital. It's at this point where Rory saves Brian Gibbons from being hit by a news van traveling at high speed. When the news van stops it puts a hole in its petrol tank, causing the petrol to run down a pipe past Kat's car. Fire men work on getting Kat out of the car, but when a fireman attempts to remove the log with the jaws of life, but suddenly, the airbag deploys accidentally and forces Kat's head into the large sharp pipe, instantly impaling her. The cigarette Kat was holding drops out of her dead hand and it blows into the pipe where the petrol spilled, igniting the petrol and causing the news van to explode. The explosion sends a barbed wire that was attached to a nearby fence at Rory, cutting him into three parts and severing his left arm. Moments after Kat's and Rory's deaths, Thomas, Kimberly and Clear all go to the hospital, where Isabella's baby is born. They think they have defeated death, but Kimberly has another vision, and realizes Isabella was never supposed to die in the pile-up. Clear then runs to find Eugene, and finds him in a hospital room. She opens the door, which pulls the plug out of an outlet, causing a spark, and an explosion, which expands through the hallway, killing Clear to the last since deaths of Flight 180 survivors after from Tod to Alex, and also Eugene, Kimberly turns around to see the dead corpses of Clear and Eugene. Working off an earlier vision, Kimberly believes that she is the "New Life", gets in a van and drives it into a lake. She drowns in the lake but is resuscitated and recovered by Doctor Ellen Kalarjian, thus making her cheat death and skipping Burke as she is the last in line. Making her and Thomas the only survivors of death's design. Sometime later, they are invited to the Gibbons' house for dinner, and they talk about how they are finally returning back to normal life. This is when Mr. Gibbons mentions how Brian was saved by Rory. Kimberly, and Burke look at each other, before the BBQ grill with Brian in front of it explodes, entirely obliterating him and his arm lands on Ms. Gibbons plate as she screams in horror. Clear then runs to find Eugene, and finds him in a hospital room. Six years after the Flight 180 explosion and five years after the Route 23 pileup, teenager Wendy Christensen and her friends go to an amusement park, celebrating their graduation from high school. Her friends decide to ride a roller-coaster called Devil's Flight. Before boarding, Kevin Fischer and Jason Wise have a coin toss to see who will sit in the front and in the back. Kevin ends up sitting in the back with Wendy, who was initially unwilling to go on the ride, and Jason, Wendy's boyfriend, sits at the front with Carrie Dreyer, Kevin's girlfriend. Upon getting on the ride, Wendy has a premonition of the roller-coaster hydraulics failing, causing the cars to derail, killing everyone onboard, including her. The vision ends and Wendy hysterically yells at the staff to let her off the ride. The attendants open the back cars, letting Wendy and Kevin off. Hot-headed fellow graduate Lewis Romero starts a fight, forcing an attendant to kick them out while several others get off the ride due to their impatience. Before the ride starts Jason tries to get off to help Wendy but the attendant ignores him and starts the ride without the others. Wendy desperately tries to stop the ride, but it is too late and Wendy's fears are confirmed when she and the others witness the cars fly off the tracks. Several weeks later, a glum Wendy is shown carrying her books out of her locker at school. Kevin tries to cheer her up, but she had refuses to go to the graduation ceremony. Two of the survivors, Ashley Freund and Ashlyn Halperin, also try to cheer her up by inviting her to a tanning salon, but Wendy turns them down. Wendy walks to her car, but is stopped by Kevin, who then tells her about the events of Flight 180. As he tells her that this could be the same thing, she responds "F**k you, Kevin" and drives off. When she gets home, Wendy looks over the photos she took with her camera on the night of the accident. Meanwhile, Ashley and Ashlyn are shown at the tanning salon. At the same time, Wendy spots some ominous things in the photos, when suddenly Ashley and Ashlyn are incinerated to death when the tanning beds unfortunately malfunction due to a chain reaction and burn them alive. After the funeral, Wendy confesses to Kevin that she now believes that Death, once again, has come back for the lucky survivors of the roller-coaster disaster. Wendy and Kevin learn that Frankie Cheeks is next since he sat behind Ashley and Ashlyn on Devil's Flight. As they pull into a fast food drive-through, a giant truck behind them starts rolling down a hill and starts heading right for them. Wendy and Kevin escape from the car before the truck runs into Kevin's car, causing the motor from his car to fly forward into the car in front of his and the fan slices a chunk of Frankie's head, splattering his brains all over the place. Wendy and Kevin look in horror as they realize that the driver was a half-headed dead Frankie. Wendy and Kevin learn that the clues to the survivors' deaths are hidden in Wendy's photos taken at the night of the roller-coaster disaster. Wendy and Kevin realize that Lewis is next and finds him exercising at a weight room. Lewis refuses to believe that Death is after him as he claims that "Death fears him". While exercising with some weights, two model swords cut the safety wire from the weight machine. The three are unaware of the severed safety wire and Lewis lifts the two weights. Without the wire holding them the two weights fall, the bowflex machine ultimately falls and crushes Lewis' head, spraying his blood on Wendy and Kevin. Afterwards, Wendy realizes that Ian McKinley is next. Ian and his girlfriend, Erin Ulmer, are working at a hardware store and Wendy and Kevin arrive to warn them. Ian and Erin scoff at the idea, just when Wendy figures out the clues to Ian's death and pushes Ian out of the way before he is impaled by wooden stakes and plywood. After Ian is saved death moves on to Erin as she falls back onto a nail gun, activating it and shooting nails through her head repeatedly, killing her and leaving Ian devastated. Later, Kevin is shown working as a security guard at the 300th Anniversary McKinley Tri-Centennial Fair. Wendy, at her house, tries to look for clues to prevent Kevin's death and her death. While looking through more photos, she is horrified to discover that her sister, Julie Christensen was on Devil's Flight and is next on death’s list. Kevin finds her at the fair when a white horse breaks loose and the rope on its neck wraps around Julie's neck. The horse gallops, dragging her all over the place, while Wendy and Kevin try to save her. Julie is nearly killed by a harrow, but Kevin intervenes by cutting the rope, saving her and skipping her on Death's list. Wendy arrives and asks her who was sitting next to her on Devil's Flight. Wendy's question is answered when Julie's best friend, Perry Malinowski, is impaled by a flagpole launched by a rope from the horse like a missile launcher. The people at the fair panic and flee from the gruesome sight. Death then moves on to Kevin, but his death is intervened as Wendy pulls Kevin out of the firework's path, nearly burning his face. As the crowd flees, Ian McKinley arrives and is shown to have gone mad after Erin’s death. Wendy then realizes that on the night of the roller-coaster disaster, she was wearing a McKinley High School shirt, leading her to believe that Ian causes her death. Wendy tells Ian to stay away, but he continues to block their escape. As Ian, believing that he is safe since Death has skipped him including Julie and Kevin, wants it to end on Wendy as he blames her for Erin’s death. Wendy then sees that the fireworks are actually going to kill her. She ducks under the fireworks which pass Ian and hit a giant cherry picker. Ian laughs while claiming that he is invincible. He shouts "I'm not gonna die! It's you, Wendy! YOU'RE DEAD!" However, after he says that, the cherry picker begins to fall. Ian looks up and the cherry picker falls on Ian, crushing and bisecting him, with the last of his strength he sticks out his middle finger. Wendy believes that Ian inadvertently took her place in death's list, which leads her to believe that it is all over. Wendy tells Ian to stay away, but he continues to block their escape. Julie and Kevin then meet Wendy on the subway, when suddenly Wendy has another premonition of the subway train crashing, killing everyone including Julie, Kevin, and Wendy. Wendy then snaps out of her vision and tells Kevin about the doomed train. She and her friends try to stop the train and escape but it's too late, the train is already in motion. Wendy's panicked face is shown through a window seconds before the screen fades to black followed by the sound of screeching metal, leaving their fates unknown, and ending the film. Eight years after the explosion of Flight 180, seven years after the Route 23 pileup and three years after Devil's Flight derailment, college student Nick O'Bannon has a premonition of a car crash while watching a race at McKinley Speedway for a study break, which sends debris into the audience, crushing some spectators and resulting in the stadium partially collapsing, killing almost everyone present in the 180 section. In a panic, Nick convinces his girlfriend Lori Milligan, and friends Hunt Wynorski and Janet Cunningham to leave, with the quartet being followed by a handful of others who become angry with Nick after he pushes past them to escape. A security guard named George Lanter intervenes when everyone begins to argue outside, just as the catastrophe Nick had foreseen occurs. Minutes later, a flying tire flies out of the race track and obliterates Nadia Monroy. After a memorial service at McKinley Speedway, Carter Daniels attempts to burn a cross on George's front lawn as he blames him for his wife’s death, but his plan backfires, he is set on fire and violently dragged down the street by his own tow truck, he is then blown apart by it and his blown off head lands right next to George who goes to see what is happening. The next day Samantha Lane is killed when a lawn mower rides over a rock, thrown in its path by her sons which is then shot right through her eye. Before their deaths, Nick had seen omens of clues of how they would die. Hearing about Carter's and Samantha's deaths on the news, Nick and Lori begin doing research, and learn about the disasters that occurred in the previous films and discover that the survivors who were saved by premonitions began dying in a series of improbable accidents shortly afterwards. While Hunt and Janet refuse to believe them, Nick and Lori manage to convince George that Death is after them and the trio begin trying to warn the other remaining survivors. While Hunt and Janet refuse to believe them, Nick and Lori manage to convince George that Death is after them and the trio begin trying to warn the other remaining survivors. Nick, Lori, and George head over to Grandstate Customs to warn Andy Kewzer that he is next to die. Andy fails to believe, but moments later he is struck by an oxygen tank, and diced into a metal grid fence. After Andy's death, Nick goes to save Hunt, while Lori and George go to save Janet. They are successful in saving Janet from a malfunctioning car wash, but Nick is too late to save Hunt when his organs are sucked out of his anus by a pool drain. Nick then believes that saving Janet had broken the chain, and Lori and Janet celebrate by going to see Love Lays Dying in a mall. Nick then learns that another survivor, Jonathan Groves was found in the debris of the racetrack, and rushes to the hospital along with George. They fail to save Jonathan from being crushed by an overflowing bathtub above his hospital room, and moments later, George is hit by an ambulance. Nick then realizes it's up to him to save the girls, and rushes to the mall. Once Nick arrives, he has another premonition showing him that Lori and Janet will die while watching the film in the shopping mall cinema after an explosion in a room behind the theater. Nick rushes to reach them, while Lori begins spotting omens warning her that the danger is not over. Once Nick arrives, he and Lori attempt to convince Janet to leave, but are unsuccessful in their efforts. Janet is killed in the explosion by a piece of shrapnel and Lori is squished to death by the gears of a malfunctioning escalator while escaping. But it all turns out to be part of a long premonition, Nick then realizes that the event hasn't happened yet but is unable to save George when he dies just as predicted. Nick hurries to the cinema, and despite Death pinning him to a pillar with a nailgun, he is able to save his friends by extinguishing the fire by triggering the fire sprinklers that would have caused the initial explosion. Two weeks later, Nick wearing a cast on his right arm, notices a loose pylon outside of their hangout and warns the workers about it before meeting Lori and Janet inside. Moments later he deduces that the mall disaster premonition was merely a red herring meant to lead them to where they needed to be for Death to strike. At that moment, the pylon collapses, causing a truck to swerve and crash into the restaurant, then running over and finally kills the last three survivors Nick, Lori and Janet together. As the final scene turns into a x-ray scene, Janet is shown to have been crushed under the tires while Lori's neck is snapped, decapitating her, and Nick is thrown into a wall, bashing his skull and jaw, sending one of his teeth flying into the camera before the credits begin to roll. The film begins with a group of workers who are gathering together for a company retreat. Sam Lawton, a fellow office worker and short order cook for the French restaurant chain Le Café Miro 81, has prepared the breakfast before the bus ride. His best friend Peter Friedkin is awaiting everyone else's arrival for the trip. Molly Harper, Sam's girlfriend, arrives and she then breaks up with him due to his dreams of going to Paris to be an apprentice for his mentor. Sam is hurt by Molly's decision. Meanwhile Peter's girlfriend Candice Hooper, an intern for the company and a gymnast, comes along with her rival Olivia Castle, a scantily clad office girl. Molly is being hit on by Isaac Palmer, a co-worker and womanizer. Dennis Lapman, the company's boss, orders everyone to move it onto the bus. Sam then goes to the construction wing of the building to find his other friend Nathan Sears, a supervisor who has a hard time dealing with construction worker Roy Carson. Nathan and Sam regroup and board the bus to their destination. Sam then has a premonition of the North Bay Bridge collapsing, killing everyone except Molly, who did not die in the premonition. When Sam comes to, he sees he is still alive and he frantically orders Molly and the others off the bus. Though confused, Peter, Candice, Olivia, Nathan, Isaac, and Dennis soon follow suit. The bridge begins to collapse and Sam rushes all of his friends to safety. The group looks surprised and confused as to how Sam knew the accident was going to happen. Jim Block is covering the case of Sam and his friends, but he finds Sam's claims hard to believe.During the company funeral, William Bludworth, a local coroner, is present and he warns the group that Death is after them. They all ignore his warnings and move on with their lives. Sam and Molly reconcile later that night. The next day, Candice is with Peter at her gymnastics practice, and he cheers her on. Candice does some work on the balance beam, and a screw from a malfunctioning fan falls on the beam. Candice does not notice the screw and has many close calls in which she almost steps on it, but it never injures her. Finishing her practice on the balance beam, Candice then goes to do some vault work. She is flipping while a fan set up by one of the coaches is turned on on the floor. Candice is almost electrocuted when the faulty cord for the fan is shown passing through a small puddle of water, most likely from the faulty ventilation. However, she places a small towel on the water before stepping on it, bypassing this threat. Another girl ascends to the beam. During her practice, she steps on the screw and falls. She stumbles around, clutching her injured foot, and knocks over a bowl which is full of powdered chalk. The powder is then blown by the fan into Candice, disorientating her. As she flips, she accidentally lets go, falling and landing on her neck. Her spine completely fractures and she folds over backwards, with some other bones popping through her skin, killing her instantly. Sam consoles a depressed Peter. The next day, the group reunites in the office, where they drink alcohol to relax. Isaac is going through dead co-workers' desks, and he comes across a coupon for a Chinese massage parlor. He soon arrives, flirting with the workers. He insults a Buddha statue, telling him to cut down on the rice cakes. Believing he is about to receive a massage from an attractive young woman, Isaac is surprised when it is revealed an older woman will be servicing him. She avoids speaking in English for the most part. After a small massage and painful chiropractic, she performs acupuncture; leaving Isaac in a room with soundproof walls and telling him to sleep. The tip of a burning stick of incense falls off onto a towel, initiating a fire. Trying to escape it, Isaac attempts to roll off the bed. However, the leg breaks, and he falls to the ground. The acupuncture needles penetrate his body and face, and a bottle of flammable disinfectant is knocked off a shelf from the impact and begins to spill. After excruciatingly pulling a needle out, he staggers towards the door. Alerted by the sound of his phone going off, Isaac watches in horror as the vibrating phone pushes a candle off the shelf into the flammable liquid - immediately igniting. He stumbles backwards against the wall. Believing he has escaped Death, his relief is quashed as a shelved Buddha statue falls and crushes his head; instantly killing him. Bludworth, who has been present for all of the deaths so far, is there and tells the remaining survivors that Death is hunting them because Sam shouldn't have had the premonition of the bridge collapse. To assuage the situation, Sam reveals that he saved Molly in his vision, so Death is not hunting her down. Peter assumes that Sam saved her at the expense of everyone else and gets mad at Sam and at the universe for saving Molly and damning Candice. Bludworth then reveals that there is another way to avoid Death. They would have to kill someone to gain their remaining days on Earth. Peter's sanity takes a plunge as he becomes obsessed with this idea and the fact that only Molly is safe. Olivia nervously goes to get eye surgery on the same day, with the doctor strapping her head into place. Olivia, fearful, clenches a teddy bear to calm herself down. As the optician prepares for the surgery by fitting a speculum on her eye, one of the teddy bear's eyes pops off due to Olivia holding onto the bear too tightly. The doctor ignores her plea for him to stay to correct her incomplete file. A bubbling water cooler knocks over a full cup of water, splashing the cord for the machine. The machine malfunctions and starts up, with the laser intensity increasing above the recommended maximum. Olivia realizes this and calls out, to no avail. She then tries to reach for the remote to control the machine, panicked. The remote falls onto the start button, and the laser slices Olivia's eye open. The laser goes off a few more times and it burns through her hand and scars her face as she attempts to shield herself, also pulling at her restraints. She finally escapes the machine, burnt and unable to see due to the surgery not having taken place and her throwing away her glasses. Sam, Molly and the doctor run in to find a terrified Olivia, injured and traumatised. Before she can be saved, Olivia trips on the teddy bear's eye which causes her to stumble back, crash through the window and tumble to her death. After falling multiple stories, she lands face-down onto a car's windshield. Her only unscathed eye pops out of its socket, rolling onto the road - a passing car runs it over.Meanwhile, Nathan is working in construction, and he is arguing with Roy, a disgruntled employee. Nathan concernedly watches as a faulty crane moves closer to the pair. He urges Roy to move but accidentally pushes him backwards as the floor gives way and the hook falls, bringing Roy down with it. Although he is stopped from landing on the spikes below, the hook initially impales Roy through the chin, and then his brain. As the group reunites, Peter finds that Nathan accidentally killed Roy; taking his life expectancy and skipping him in Death's design. Meanwhile, as Dennis grills them all over Roy's death, a stray wrench that Roy placed on a belt sander earlier is propelled into Dennis' eyes and through his skull, killing him instantly. Later that night, at Le Café Miro 81, Sam's mentor approves his apprenticeship in Paris. He also lets Sam have the restaurant for the night to spend it with Molly. Peter crashes the dinner, and he tells of how he tried to push a woman in front of a truck to steal her life. Peter admits he couldn't do it, and he then ponders Candice's death and how she didn't deserve to die, but yet Molly supposedly survived Sam's vision. Peter then tries to kill Molly. Molly takes refuge in the kitchen and Peter knocks Sam unconscious with his gun. Agent Block arrives and hears the first gunshot as Molly hides. He is shot in the back by Peter. Molly begs Peter to leave, as he has taken Block's lifespan. However, Peter does not want to spend the rest of his newly earned life in prison. As Peter pursues Molly, Sam wakes up and fights him off. In the struggle, Peter's gun is thrown onto the burning stove, aimed at Sam, then Peter beating him for a brawl. As Peter is about to kill Molly, Sam stabs him in the back with a large skewer. Sam realizes he stole Agent Block's life from Peter, with Peter's gun finally shooting and missing Sam as confirmation. Two weeks later, Sam and Molly are boarding a plane to Paris, and she asks for the window seat. They see two high school students fighting and getting kicked off the plane. This group is recognizable to the audience as Alex Browning and his class. Sam and Molly are on Flight 180 - the film is a prequel to the first Final Destination. Sam and Molly believe that the kid is just having a panic attack. During takeoff, Sam overhears a flight attendant tell a passenger that Alex claimed to have "some kind of vision". Sam exclaims in horror as Alex's premonition begins to come true. As the engines start to disintegrate and oxygen masks dangle from the ceiling, Molly is sucked out of the cabin and is bisected by the plane's horizontal stabilizer. Seconds later, the rest of the plane is engulfed in flames and explodes, burning Sam and everyone on board to death. As the plane explodes, the flame-engulfed landing gear is blown away and tumbles towards the city. Nathan is at a local bar for Roy's memorial service. There, he talks to a coworker, who explains that Roy's autopsy revealed he had an enlarged blood vessel that would have burst "any day now". Nathan realizes with dread that now he could be dead or back on the list at any second. Before he has time to process anything, the landing gear from Flight 180 crashes through the bar and crushes Nathan, launching his severed hand into the camera. The film proceeds to cut to a collage of previous disasters and deaths scenes from the first four films. Plagued by a violent recurring nightmare, college student Stefani heads home to track down the one person who might be able to break the cycle and save her family from the grisly demise that inevitably awaits them all.In 1968, Iris Campbell and her fiancé Paul attend the opening ceremony of The Sky View restaurant. At the dance party, Iris has a premonition in which a chandelier's shard cracks the glass floor beneath the guests while a gas heater leaks, exploding the tower and killing everyone. In the present fifty years later, college student Stefani Reyes suffers from a series of nightmares about the collapse. She returns home seeking answers and is greeted by her father, Marty, and her estranged brother, Charlie. The siblings visit their uncle Howard, aunt Brenda, and cousins Erik, Julia, and Bobby. When Stefani asks Howard about Iris, her maternal grandmother, he explains that Iris subjected him and Stefani's mother, Darlene, to an overprotective upbringing, then became reclusive, causing child services to take him and Darlene away and the latter to abandon the Reyes family after emulating her mother. Brenda gives Stefani letters from Iris, leading her to a fortified cabin in the woods where Iris now lives.Terminally ill, Iris tells Stefani she disrupted Death's design by warning Paul and the guests to leave the glass floor before it broke. As a result, the tower never collapsed, but Death began killing the survivors and their descendants, who were never meant to exist, including Paul, who died in an accident, prompting Iris to document Death's omens in a book. Though Stefani is skeptical, Iris follows her out the cabin to give her the book and prove her claims by letting a chain reaction cause a weather vane to fatally impale her through the mouth. At Iris's funeral, Darlene returns, but Stefani resents her for being absent since childhood. At a family barbecue, Stefani reads the book and finds a reference to someone identified as JB. During the event, Howard steps on a glass shard and falls into the path of a lawn mower, killing him. Later that night, a chain reaction causes a fire at Erik's tattoo shop, but he surprisingly survives due to his leather clothing. As Erik boasts to Stefani about it, Julia gets knocked into a trash bin and is thrown into a garbage truck, where she is crushed to death. The family realizes Death is targeting Iris's bloodline in order of age. Marty and Brenda are spared, as they are not blood relatives, and Erik is skipped due to being the result of Brenda's affair. Darlene suggests visiting JB at a hospital, who turns out to be William Bludworth. Present at the tower as a child and also terminally ill, he warns the family of the consequences cheating Death and explains two ways to defeat it: taking another life or dying and being revived. Erik gives Bobby a snack containing nuts, intending for Bobby's allergic reaction to kill him. Erik brings in a wheelchair and plans to use it to rush Bobby to the ER so he can be revived by medical personnel. The plan fails when an MRI machine overloads, pulling Erik's body piercings and crushing him inside with the wheelchair. The machine then sends a vending machine dispenser coil to impale Bobby's head. Stefani, Charlie, and Darlene flee to Iris's cabin in Darlene's RV, intending to hide Darlene from Death. The RV crashes through the gate, causing Stefani's seatbelt to jam. The cabin explodes after a chain reaction, sending the RV into water and Stefani begins to drown. Darlene saves Charlie from the wreckage and ushers him to rescue Stefani before a lamp post falls and bisects her. Charlie breaks the seatbelt and successfully resuscitates Stefani with CPR. A week later, before Charlie's senior prom, the siblings realize Stefani was only unconscious during the drowning. At that moment, a train derails into the neighborhood, and several logs fall onto Stefani and Charlie, killing them.
