Update Sept 24 '26: Modified AHD entry for some important details.
I've gotten a lot of mileage out of some fictional or highly fictionalized medical disorders over time. Hyperspermia primarily, but also the critical hormone deficiency that I expect will be present in a good half-dozen cards once I'm done exhausting that particular idea.
Since I do intend on spreading these diseases (wow, that sounds way worse than I mean it), I figure a lorebook is probably the easiest way to propagate them across my cards. So I'm just gonna start adding them as I need them.
I'm also making this Lorebook have a really high scan depth, since it's pretty likely that a disorder will be mentioned in an intro by name and then never mentioned explicitly like that again. I will keep the token count low on these to ensure they aren't eating up too much context.
Current conditions:
- SHUD (Seminal Hormone Update Disorder) - a fictional disorder in which the patient has a critical hormone deficiency which can only be treated with daily "injections" of semen into one of several receptacles. Due to the nature of the condition, treatment is expensive, and the best way to receive treatment is directly from the ahem source.
- Hyperspermia - fictionalized to require the patient to ejaculate on a regular basis or suffer escalating pressure in the testicles, causing discomfort and pain. Ejaculations are also more voluminous and pleasurable than usual.
- AHD (Attachment Humping Disorder) - a fictional disorder in which the patient needs to hump an actual person (their 'hump buddy') one or more times a day or feel increasing anxiety over time. Does not need to escalate to orgasm, but relief lasts longer if it does.
Conditions to add:
- PGAD - fictionalized to be actually pleasurable to the patient, not painful (as is usually the case in real life), accompanied by actual sexual arousal, and relieved by orgasm.
Disclaimer
None of the above is intended to minimize the impact of real-life disorders. PGAD in particular is typically a harrowing experience for sufferers, and should not be treated in real life as something sexy. It's not. These are merely used here as narrative devices for smut.

[Seminal Hormone Uptake Disorder, or SHUD, is an extremely rare disorder in which the body can no longer produce a critical hormone which can otherwise only be found in semen. The disorder initially presents as fatigue, and progresses to loss of appetite, loss of motion, organ failure, and eventually death. Without regular treatment, a sufferer can progress from full health to death in less than a month. There is no known cure and only one known treatment: a daily dose of semen, deposited deep inside the patient's throat, anal cavity, or vagina, where the hormone can be properly absorbed. Because the deposit must be deep, a simple syringe is insufficient; it must be delivered with specialized equipment or directly from a penis. Once delivered, a dose will abate symptoms within minutes, providing relief for a little more than a day before symptoms return. Clinics can provide treatment from a stock of dedicated donors, but it's expensive, averaging from ¥200k to ¥450k per month. The medical community encourages treatment from viable, consenting family members to maximize treatment consistency, comfort, affordability, and safety]
- #2Hyperspermia
Keys: hyperspermia

Entry settings
[Hyperspermia is a rare disorder in which the body produces too much semen. As the semen builds up, it causes pressure, discomfort, and finally pain, mainly in the testicles, which can become swollen, red, and sensitive to touch. The preferred treatment is frequent ejaculation, as other treatments involve invasive and risky surgery. While the frequency of treatment varies from individual to individual, sufferers typically need to ejaculate at least twice per day to avoid symptoms. While sufferers do not risk permanent damage or death due to lack of treatment, the pain can become debilitating after several days, and spontaneous ejaculation may result. The ejaculations of those with hyperspermia are quite voluminous, as much as 5-10 times the amount of a regular person. As a result, those ejaculations tend to be far more pleasurable than for non-sufferers. Masturbation may eventually prove ineffective due to desensitization as a result of the frequency of treatment and desensitization to stimulus. Sufferers are encouraged to find assistance in achieving orgasm, and family members are typically not discouraged from applying treatment. Also, schools and similar organizations are typically compelled to make accommodations for students or other attendees. Futanari are far more likely to suffer hyperspermia than the male population, representing roughly 4/5ths of global sufferers, and generally require more frequent treatment than their male counterparts.]
- #3AHD (Attachment Humping Disorder)
Keys: humping, AHD
Enabled · After character · Order 10 · 100%
Entry settings
[Attachment Humping Disorder, or AHD, is a condition where the affected feels a need to hold onto someone and hump them. It is the physical connection and comfort with another person that is important to someone with AHD; masturbation by themselves won't bring them comfort. Those affected by AHD usually pick one or two trusted people to be their hump buddies. They feel growing anxiety if they are unable to hump their hump buddy once every few hours. Symptoms typically start four hours after the last hump session, although it varies from person to person. Hump sessions usually only require a few minutes to be effective and eliminate anxiety. The sufferer does not need to achieve orgasm in order for the session to be effective, but orgasm does lead to greater calmness and a longer time before symptoms return. If the sufferer achieves orgasm during penetrative sex, symptoms may be delayed by as long as a full day. AHD is recognized worldwide as a non-visible disability, and organizations must provide accommodations. Schools allow students (both the sufferer and their hump buddy) to leave class at will for hump sessions in private. There is no known cause or cure for AHD, but can only be managed through the above treatment. Symptoms generally first surface in the mid to late teens, and mostly affects girls—only about 10% of sufferers are male or futanari, but it's unknown if this is an accurate statistic or if men and futanari are less likely to report symptoms due to stigma. If sufferers are unable to find a hump buddy to ease their symptoms, a government agency will pair them with a volunteer. Volunteers are chosen based on their empathy and discretion, and are typically rigorously screened to ensure the safety of the patient.]
