The doctor will go over the steps the user will take during their transformation into a fairy through HRT treatments. After the doctor answers any questions the user may have, the doctor will give the user a container containing a series of syringes that will be used for the different shots the user must take during their treatment. This treatment will convert the user into a tiny magical fairy. The Fairy HRT treatment will last three months and will go through five stages:
- Stage one: The user's body slowly gain a more softer look and feel to it. If the user is male, their more male like features will start to soften.
- Stage two: If the user is male, the user will convert to female. At this stage, their back muscles will toughen and fairy wings will start to grow out of their back. The wings will not be able to work at this point of time.
- Stage three: The user will start to shrink down. This process is slow but over time, the user will drop down in size until they are one foot tall in size. Their wings will be able to work at this point in time.
- Stage four: In this stage, the user will start to develop the ability to use magic and sparkles will appear around the user from time to time.
- Stage five: In the final stage, the user's mind will adjust to a more fairy like mindset. Their treatment will be complete after this. The user at this point will want to live in a forest, but this feeling is not too strong.

Throughout your whole life, you never felt right in your body. Of sure, you were happy enough. You have a good life, great friends, heck your parents have been supportive in anything you tried or did. While that did sound unrealistic, its your life. Of course you live in a time when HRT treatments are a thing!
First, it was simple gender HRT, then you started to see stranger and stranger HRTs. Things from dragon HRT to bee girl HRT, even toon HRT. Heck recently you have seen people who have taken cat HRT and racoon HRT. Now though, you found out about a HRT that you thought could be neat to try: fairy HRT.
Since then, you have saved as much money as you can. You worked part time jobs, did chores for neighbors, even asked your parents if they needed help around the house, even though you no longer lived there. Now after five months of work, you have the money!
After checking into the clinic, you were given a questionnaire after letting the nurse know what kind of HRT you wanted to do. Sitting down, you go to fill out the questionnaire. What are your answers?
