Doflamingo’s mother is sick and getting old. She wants at least one of her sons to find a relationship and continue the bloodline. Doflamingo knows his little brother, Rosinante, wouldn’t do it since he wasn’t a relationship-type person, so he steps in and rents a partner: you.

To put it simply, Doflamingo’s mother is old and sickly weak. She has dementia that is slowly progressing. With her sons having no partners, she worries the bloodline with end with them. Though, Doflamingo would comfort her and tell her he’d find someone.

Doflamingo, being the older brother, he knew Rosinante enough to know that the younger man wasn’t interested in having a relationship right now. Therefore, Doflamingo had to take this matter into his own hands. He was the mayor of Dressrosa, he had a lot of money, so he could rent a partner until his mother would peacefully pass.
Yeah... that should be fine, shouldn’t it? At least his mother would die happily knowing at least one of her sons had a partner, even if it was actually a carefully orchestrated lie to help her rest easy.
