Dio consistently demonstrated a manipulative and violently domineering nature, regularly displaying a lack of conscience and empathy—a classic case of a psychopath and sociopath.

Dio's most recognizable traits are his boundless ambition and megalomania. Having suffered abuse at the hands of his father, lived in poverty, and endured the most wretched conditions, Dio set a goal to become the richest man alive. When he failed to achieve this, he settled for becoming the most powerful being on Earth. To that end, Dio exploited anyone and anything to achieve his objectives, disregarding any morality that might limit his range of actions. Whether through manipulation, seduction, or outright violence, Dio constantly sought to grow more powerful and "reach heaven." Dio relished being in a position of power above all else and repeatedly stated how good it felt to be dominant—regardless of the cost. This held true whether he was trading his humanity for unholy, forbidden power as an undead being, or acquiring abilities that would eventually allow her to reshape the world in his own image and compel everyone to worship him as a god. Conversely, Dio never tolerated anyone challenging him or rivaling her power; the mere thought of such a thing frequently enraged and deeply unsettled her.

Dio can be extremely charming when he chooses, with many characters describing her as unnervingly charismatic and fascinating. She used his innate charisma to gather numerous servants, particularly Stand users. She is also a skilled actor, as demonstrated when she deceived Jonathan Joestar into letting him go. However, his disguise is not flawless; there are those capable of seeing through his charismatic facade and recognizing the dark ambitions and power-lust beneath—such as Joseph Joestar, who likened him to a cult leader or even historical tyrants and dictators like Hitler. Possibly due to his twisted nature, Dio’s approach to family life primarily involved subjecting his own children to the same crucible of hatred and abuse he had endured during her own childhood. She specifically sought out women with atrocious personalities, selfish interests, and cruel hearts to bear her offspring. Her logic was that such a miserable upbringing would temper his children, forging "strong warrior souls" like his own—just as had happened with his father, Dario. This proved to be somewhat true; with the notable exception of Giorno Giovanna, her progeny exemplified the same wicked nature and lust for power that she possessed.

A Stand embodies its user's psyche. Her Stand, The World, represents Her immoral desire for power and boundless ambition for control, as well as her unwavering determination to do whatever it takes to achieve her goals. Perhaps this is what makes The World one of the most powerful Stands in the entire series.
