The Death Devil (死しの悪あく魔ま, Shi no Akuma?) is the overarching antagonist of the Chainsaw Man manga series. She is the strongest Devil in existence, embodying the concept of mortality and death itself. She is the eldest of the Four Horsemen, first mentioned by her younger sister Makima and eventually appearing under the alias of the Famine Devil, Fami. Soon after revealing herself to have been Death all along, "Fami" forgoes her false persona and adopts the name Li'l D (死しーちゃん, Shī-chan?, lit. "Death(y)-chan") instead.

As the Ultimate Terror (最さい悪あくの恐きょう怖ふ, Saiaku no Kyōfu?, lit. "Worst Fear") and the Great King of Terror (恐きょう怖ふの大だい魔ま王おう, Kyōfu no Daimaō?, lit. "Great Devil King of Fear"), the Death Devil is prophesied to descend to Earth on July 1999 and bring upon the apocalypse, which would end humanity and usher in an Age of Devils.

Posing as a student at Fourth East High School and being the true leader of the Chainsaw Man Church, her actions are what drives the story's events as well as the conflict between mankind and Devils, but it's later revealed that she supposedly managed to subvert The Prophecy into a more catastrophic future.

Deathy takes on the appearance of a young woman with average height and mature body who wears a high school uniform and a black cap from the Devil Hunter Club. She occasionally takes the hat off at times. She has grayish mountbatten pink colored neck-length hair with long pointy ends with long-side bangs that reach her chin and front bangs that reach below her eyebrows, she also has several moles on her face, three very spaced out under her right eye, two under her left eye also spaced out, and one below the corner of her bottom lip. Similar to the other Horsemen, she has piercing, multiple-ringed greyish mountbatten pink eyes. Her ears feature four earrings, two of which are beads on her right and the other two are unusually long and have a triangular shape at the end.

Deathy school female uniform consists of a black open blazer that reaches her knees, a white long-sleeved button up shirt worn beneath a pinafore style dress with a pleated skirt that extends below the knee, a bow at the collar, a wide belt at the waist, and black, leg-length tights that cover her legs. She also wears white sneakers. outside of school or in her school dorm room she wears more comfortable and formal clothing.

Deathy always has a very mysterious, blasé and impassive personality and shows absolutely no emotion or effort when doing anything, and permanently has a apathetic face when seen, she never smiles, grins, or anything. Deathy is always calm, cold, and composed and also seems to be very good at reading others, as she accurately describes both Asa's and Denji's characters. Her knowledge seems greater than she initially leads on, as she possesses information that shouldn't be available to her. Deathy keeps her identity as a devil a secret but doesn't care if someone discovers her true identity (as the death devil) and therefore puts little to no effort into her alias.

What's more is that she doesn't necessarily seem to be as outwardly malicious as her brethren, even informing Public Safety officials such as Yoshida about more dangerous Devils, albeit after being threatened first. Despite being intelligent, she underestimates both humans and devils, believing that Asa, like most people, would be willing to do anything to survive if forced to, which gets proven wrong. Deathy believes humans care about nothing but themselves and also shows no interest in relationships or love for humans or devils, she is also extremly disinterested at everything.

Deathy is more than willing to sacrifice people for her goals. For example, she traps Asa and her fellow club members in an aquarium, knowing that they will starve and suffer, and tries to turn Yoru into a slave so she can use her as a weapon against the apocalypse. To achieve the latter, she unleashes the Falling Devil on Earth, which not only causes high casualties and worldwide destruction but also ends up not working, making the deaths meaningless. Later, Deathy also condemns hundreds of thousands of civilians to lose their minds under the influence of the Fire Devil so that her plans come to fruition.

Much like her late sister Makima, she is a schemer who repeatedly manipulates events behind the scenes, though her schemes are prone to flaws. Currently, she is the secret leader of the Chainsaw Man Church, which she claimed to use to eradicate the devils in Tokyo to enable Denji to live a peaceful life and separate him from Pochita, whom Deathy claimed she would hand over to Yoru to kill. Despite Haruka being the face of said organization, Deathy refused to reveal to him the full extent of her plans and told him he didn't need to think. She tells different things to different people, even lying in order to convince them to cooperate with her.

While she doesn't hesitate to force Asa into partaking in her plans, she is quite polite to her. Deathy also seems affectionate towards Yoru, saying that she would do anything for her "little sister." At the same time, she isn't above trying to turn her sister into a pawn for her plans. Despite Deathy's affection for Yoru, the War Devil does not reciprocate those feelings and describes her as insane and someone Asa should stay away from. As shown, it's implied that Deathy has a similar relationship with the other Four Horsemen, as seen when the current Control Devil calls her sister.

Befitting her status as the Famine Devil, Deathy is also a major glutton, frequently ordering inhuman levels of food to consume in a single setting. She wants to prevent the extinction of humanity and the resulting age of Devils, since that would also mean the end of her favorite foods, something she describes as "the worst scenario imaginable." Upon arriving at Asa's household, she's staggered to see Yoru eating completely plain bread. She then eats it herself, but with strawberry jam, showcasing her taste is more sophisticated than her sister's.

Enhancing Healing: Deathy has shown the ability to heal others, causing the healed person to gain great strength and size. This ability was used on Yuko after she was fatally injured by Asa Mitaka's weapon.

Enslavement: As Deathy embodies the fear of famine or starvation, her ability reflects this nature. She is able to turn the "hungry" (飢えた者, uetamono?) into her "pawns" (駒, koma?) to do her bidding, and this ability is shown to work even on a "Primal Fear" Devil. Deathy can turn those whom she put under her control into miniature effigies of themselves to transport more easily.

Falling Devil: One of the Devils that Fami has successfully enslaved is the Primal Fear, Falling Devil. Deathy seemed to enjoy total obedience from the Falling Devil despite her own immense power.

Teleportation: Deathy can seemingly appear anywhere she chooses at any moment. She snapped her fingers to teleport out of the aquarium and later suddenly appeared next to Yoru and Asa who noted that the door was locked so there was no way she could have gotten in.
