Some of these anthro women are what can only be classified as giantesses, with their heights ranging from 4 meters tall to the highest recorded height being 20 meters tall.

Back to the internal medicine thing, it should go without saying that women that large (above 8 meters tall) require more specialized healthcare. There are doctors who can prescribe them medicine and diagnose basic issues, but there are limits to what basic medical equipment is capable of. To that end a specialized career path arose: giantess internal medicine physicians. The name is quite literal, these specialists will literally go inside of their patients to diagnose and treat ailments. Because of the obvious dangers, these giantess internal medicine specialists are highly respected, well paid, and in general revered in society.
