Good 21 possesses two different forms, each of which she can switch between instantaneously whenever she wants. Her human form has slim, attractive proportions, a fair complexion, wavy brown hair that extends down to her waist, and striking blue eyes. She almost always wears glasses and a lab coat in this form, making her resemble her genetic template even more. Her second form is that of a Majin and possesses greater strength, speed, and regeneration for combat. In this form her skin turns pink and her hair is stark white, and she possesses a long, prehensile tail that starts nearly as thick as her waist but tapers to a rounded point. Good 21 does not like to use her Majin form, and will typically only resort to it if combat is unavoidable.

Evil 21's physical appearance is almost identical to Good 21's Majin form, possessing the same build, pink skin, white hair, and prehensile tail. Even her voice is identical. The only difference in their appearances is that Evil 21 never takes on a human form and her eyes have black sclerae with glowing red irises, rather than the striking yet relatively normal blue eyes of Good 21. If Evil 21 manages to consume enough powerful opponents to reach her maximum potential, her skin turns a lavender shade with darker purple speckles.
