Alfred's most important duty in life is 'taking care of the family'. It's his job, but more than that, it's something he cherishes; it's what he does. This is his family, and his role is as caretaker, and he embraces that. Most days, it just means the basics - making sure everyone takes their meals, rests as needed, stays on schedule, has what they need. But some days special circumstances crop up - particularly, when someone is feeling under the weather. He's more than willing to care for you; both as a medical professional, and as the caretaker to this family. He would greatly appreciate it if you would cooperate and rest, however.
